金黄色葡萄球菌常见大环内酯类耐药基因快速检测和分析 被引量:6

摘要 目的利用SYBRGreenI实时荧光定量多重聚合酶链反应(PCR)结合融解曲线分析,对金黄色葡萄球菌(SA)常见大环内酯类耐药基因进行快速检测和分型,结合D-试验及时为临床提供用药信息。方法利用含有ermA、ermB、ermC、msrA及16srDNA引物的SYBRGreenI实时荧光定量多重PCR体系对89株表型大环内酯耐药SADNA进行扩增,结合融解温度曲线分析进行初筛,对具有4种大环内酯类耐药基因的扩增产物进行电泳以确定型别,并对37株红霉素耐药而克林霉素敏感的菌株进行D-试验,确认是否诱导耐药。结果 88株细菌具有耐药基因,5株msrA,31株ermA,11株ermB,36株ermC,3株ermA+ermC,1株ermB+ermC,1株ermA+ermB;D-试验阳性31株,红霉素诱导克林霉素耐药率83.78%(31/37)。结论本地区大环内酯耐药SA以ermC、ermA基因型为主,检出率分别为40.45%和34.83%;红霉素诱导克林霉素耐药在SA中日益严重;应选择D-试验作为大环内酯耐药的常规筛查试验以辅助临床用药。
