摘要
目的了解本院抗生素使用情况及医院感染中革兰氏阴性细菌的耐药现状,为临床合理使用抗生素提供必要的参考依据。方法回顾性调查山西省晋中市第一人民医院2006-2009年4年间抗生素使用情况及医院感染中革兰氏阴性菌分布及其耐药性的变化情况。采用世界卫生组织(WHO)推荐的限定日剂量(DDD)法分析各种抗菌药的用药频度(DDDs),用Excel进行数据编码、合并、排序、分类、统计等处理。结果①4年中抗生素总体DDDs呈平稳态势,β-内酰胺类的复合制剂的DDDs在2006-2009年逐年增长,亚胺培南的DDDs4年中上升明显,分别为89,101,136,215。②革兰氏阴性菌对哌拉西林/他唑巴坦及头孢他啶的耐药率尚保持在30%以下,但对哌拉西林/他唑巴坦的耐药率也呈上升趋势,对亚胺培南仍保持有较高的敏感性,耐药率大都在10%以下。结论细菌耐药与抗生素的过多使用有直接关系,应加强对医院感染中病原学的监测,合理使用抗生素,控制哌拉西林/他唑巴坦及亚胺培南在晋中一院的使用量。
Objective To investigate the use of antibiotics and the drug resistance of common Gram-negative bacilli isolated in our hospital for providing a basis in the proper use of antibiotics. Methods The data of antibiotics and common Gram-negative bacterial drug resistance in nosocomial infection were retrospectively surveyed and analyzed in Jinzhong First People’s Hospital from 2006 to 2009 . Frequencies( defined daily dose,DDD) and duration of antibiotic medication were evaluated according to the WHO-recommended DDD method. Results ①The total DDDs of antibiotics remained steady over 4 years,however,the total DDDs of beta-lactamase compound preparation sharply increased from 2006 to 2009. The DDDs of imipenem were 89,101,136 and 215 days in 2006-2009,respectively. ②The resistance rates of Gram-negative bacterial to piperacillin sodium/tazobactam sodium and ceftazidime were lower than 30% ,however,the resistance rate to piperacillin sodium/tazobactam sodium increased progressively in 4 years. Gram-negative bacterial were susceptible to imipenem( 10% ) . Conclusion The drug resistance in Gram-negative bacilli is directly correlated with the over-use of antibiotics. The pathogen monitoring of patients in nosocomial infection should be paid more attention. Rational use of antibiotics is important for the containment of bacterial resistance. The DDDs of piperacillin sodium/tazobactam sodium and imipenem should be controlled in Jinzhong First People’s Hospital.
