摘要
目的探讨红细胞压积变化对干血片法Down综合征产前筛查结果的影响。方法用同一份全血,通过改变血浆与红细胞比例,制备一系列不同红细胞压积的全血标本并进行甲胎蛋白(AFP)及人绒毛膜促性腺激素(β-hCG)检测,计算全血浓度/血浆浓度值并与血浆体积/全血体积值比较,分析AFP及β-hCG在血浆和红细胞中的分布情况;通过制备不同红细胞压积的标准全血和滤纸干血片,分析红细胞压积对AFP及β-hCG浓度的影响;根据红细胞压积不同将1 155例孕妇分为三组,对各组滤纸干血片中AFP及β-hCG的浓度进行比较;对1 320例孕妇同时进行血清和滤纸干血片Down综合征筛查,比较筛查结果。结果不同红细胞压积的全血标本中,AFP及β-hCG全血浓度/血浆浓度值与血浆体积/全血体积值无统计学差异,即AFP及β-hCG在红细胞中无分布;不同红细胞压积的滤纸干血片标本中AFP及β-hCG浓度值无统计学差异,而全血标本中AFP及β-hCG浓度值随着红细胞压积的变化而变化。1 320例孕妇滤纸干血片法Down综合征产前筛查结果与血清法符合率为98.6%。结论红细胞压积对Down综合征滤纸干血片产前筛查结果无明显影响,滤纸干血片标本可以替代血清标本。
Objective To explore the effects of hematocrit(HCT) on prenatal Down syndrome screening results by dried blood method.Methods A series of whole blood samples derived from the original blood were created by altering the ratio of plasma and red blood cells.The amout of AFP and β-hCG in the whole blood samples with different HCT were tested;The ratio of AFP(β-hCG) concentration in whole blood and plasma was calculated,and were compared with the ratio of the volume of plasma and the volume of whole blood,then the AFP(β-hCG) distribution in plasma and RBC was analyzed;by making the whole blood calibrator and dried blood spots which had different HCT,the impact of different HCT on AFP(β-hCG) concentration in the whole blood and dried blood spots was analyzed;1 155 pregnancies were divided into three groups according to the magnitude of HCT,and AFP/β-hCG concentration in dried blood spots between three groups were compared;1 320 pregnancies were screened Down syndrome with the dried blood samples and the serum samples,the two method screening results were compared.Results In the same samples with different HCT,no significant difference were found between the whole blood to plasma ratio of AFP/β-hCG concentration and the ratio of the volume of plasma and whole blood,there were no AFP/β-hCG in the RBC;AFP/β-hCG concentration in dried blood spots maintained constant when HCT changed,but changed in whole blood with the change of HCT.The coincidence of Down syndrome screening results between dried blood samples and serum samples were 98.6% according to 1 320 pregnancies results.Conclusion HCT has no significant influence on Down syndrome screening results using dried blood sample,so dried blood sample can substitute serum sample.
