摘要
目的探讨无创正压通气(NIPPV)在慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者中的应用与护理措施。方法 78例患者随机分为观察组和对照组各39例。对照组给予常规治疗;治疗组在对照组基础上给予NIPPV治疗及相应护理措施。2组疗程均为7d。观察2组动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、心率(HR)及呼吸(R)变化情况。结果观察组治疗后HR、R、PaCO2降低,PaO2升高,与治疗前比较差异均有统计学意义(P<0.05),且改善情况优于对照组,差异有统计学意义(P<0.05)。结论 NIPPV在COPD合并Ⅱ型呼吸衰竭患者中应用效果良好,合理的护理措施可提高患者生活质量,值得临床推广应用。
Objective To explore the application and nursing method of non-invasive positive pressure ventilation （ NIPPV） in patients with chronic obstructive pulmonary disease（ COPD） and respiratory failure of type Ⅱ. Methods 78 cases were randomly divided into control group and observation group,39 cases of each. The control group was given conventional treatment,while the observation group based on the control group was given NIPPV and corresponding nursing method. Both groups were treated for 7d. Observed 2 groups＇change situation of PaO2,PaCO2,HR and R. Results After treatment,HR,R and PaCO2 of the observation group reduced,while PaO2 rose,and compared with those before treatment,the differences were statistically significant（ P〈0. 05）. Furthermore,the improvement of these indexes zbove of the observation group was better than that of the control group,and the difference was of statistical significance（ P〈0. 05）. Conclusion NIPPV had good efficacy on COPD with type Ⅱ respiratory failure,and rational nursing method can improve life quality of patients,which is worth clinical popularizing.
