摘要
目的:与常规门静脉造影对照,评价门静脉三维动态增强磁共振血管成像(3dimensionaldynamiccontrastenhancedMRA,3DDCEMRA)显示门静脉开放性及侧支循环的能力。材料与方法:共有20例患者做了门静脉3DDCEMRA成像,分析门静脉3DDCEMRA上门脉主干、肝内门脉左右分支的开放性;侧支循环发生的部位和严重程度。3DDCEMRA的显示结果与门静脉造影术(经皮肝穿门脉造影或经脾动脉/肠系膜上动脉间接门脉造影)相对照,评价3DDCEMRA与门静脉造影术的符合情况。结果:在20例患者60支门脉主干和左右分支中,有57支两种检查方法显示门脉的开放性的结果相同,符合率为95%。但有3支肝内门脉分支,两种检查方法评判其开放性所获结果不一致。3DDCEMRA所揭示的侧支循环的分布情况则同门静脉造影术完全一致,符合率为100%。结论:门静脉3DDCEMRA同常规门静脉造影术符合率很高。
Objective: To evaluate 3D DCE MRA in demonstrating of the patency and collaterals
of the portal vein by comparing it with conventional portography. Materials and Methods:
Twenty patients underwent 3D DCE MRA of the portal vein. The patency of the main portal vein,
its right and left intrahepatic branches was examined. The distribution and extent of collaterals
were assessed. The findings demonstrated on 3D DCE MRA were compared with those seen on
conventional portography (percutaneous transhepatic portography or transarterial indirect
portography). Results: Of 60 portal veins in 20 patients examined with both techniques, 57 had
identical appearance in both 3D DCE MRA and conventional portography, with an agreement of
95%. The patency of the remaining 3 intrahepatic portal veins was discordant between two
techniques. The presence and distribution of collaterals revealed by 3D DCE MRA was
correlated exactly with portography, with 100% agreement between the two methods.
Conclusion: 3D DCE MRA facilitates quite accurate determination of the portal venous patency
and existence of varices, highly identical with conventional portography.
