摘要 目的:分析2008年广东省O1/O139群霍乱弧菌分离株的病原学特征,比较不同来源的分离株之间的克隆相关性。方法:对霍乱分离株进行血清分型,O1群菌株进行噬菌体-生物分型。采用纸片法检测菌株对11种抗生素的药物敏感性;PCR检测毒力基因;应用脉冲场凝胶电泳技术进行分子分型分析菌株克隆相关性。结果:2008年从病例及密接者中分离得到5株菌株,3株O139群,2株小川型菌株,除1株小川型菌株外,均为产毒株。常规监测分离株27株,珠江河口水体分离株28株,菌型分布散在,与病例分离株之间同源性不高。药物敏感性检测发现,菌株对诺氟沙星、环丙沙星敏感,对萘啶酸、四环素耐药性高,不同血清型菌株对抗生素敏感性不同。PFGE同源性分析显示不同来源菌株之间克隆相关性不高。结论:2008年广东省霍乱菌株菌型多样,无明显优势菌型,外环境中存在大量非产毒株,需要密切监测外环境菌株获得毒力后引起的霍乱流行。 Objective:To analyze the etiologic characteristics of Vibrio cholerae in Guangdong province in 2008.Genetic relationship was observed between the isolates from different sources.Methods: All the isolates were typed by sero-typing and the V.cholerae O1 El Tor isolates were analyzed further for phage-biotype.The antibiotic susceptibility test(AST) of the isolates was taken by K-B disc diffusion.The toxic genes were detected by PCR and the genetic relationship was determined by the pulsed field gel electrophoresis(PFGE).Results: In total,5 isolates from cholera cases and closely contact and sub typed for 2 serogroups(3 isolates of O139 and 2 isolates of O1 El Tor Ogawa).4 of the 5 isolates were toxigenic while only 1 isolate of O1 El Tor Ogawa was non-toxigenic.However,27 isolates were collected from environment surveillance programs and anther 28 isolates were collected from the Estuary of the Pearl River surveillance.These was no predominated biotype could be found in all these isolates.Additionally,high phylogenetic diversity was observed for the same sero-type isolates from cases and surveillance samples.The antibiotic susceptibility test discovered that the isolated were susceptible to the NOR and CIP while resistant to the NAL and TET.Additionally,the AST were different according to different sero-types.The PFGE analysis discovered high phylogenetic diversity among isolated from different sources.Conclusion: There was no predominated biotype could be found in Guangdong province in 2008.The amount of non-toxigenic V.cholerae isolates in environment could turn into toxigenic by obtaining the virulence genes.It is important to make a closely surveillance plan for preventing the epidemic of cholera.
