前列腺按摩后尿液中融合基因TMPRSS2:ERG亚型T1E4 mRNA的定量检测在前列腺癌诊断中的应用 被引量:1

摘要 目的:建立实时荧光定量RT-PCR检测前列腺按摩后尿液中融合基因TMPRSS2:ERG亚型T1E4 mRNA的方法学并探讨T1E4 mRNA在前列腺癌临床诊断中的应用价值。方法:前列腺癌(PCa)患者46例,前列腺增生(BPH)患者44例,收集前列腺按摩后尿液,离心取细胞沉淀物,提取总RNA,实时荧光定量RT-PCR方法分别检测PSA mRNA和T1E4 mRNA,以T1E4 mRNA/PSA mRNA比值表示融合基因T1E4 mRNA的含量。比较PCa组与BPH组尿液T1E4 mR-NA的含量(T1E4 mRNA/PSA mRNA比值),用受试者工作曲线(ROC)评价尿液T1E4 mRNA含量对前列腺癌的诊断价值,并分析尿液T1E4 mRNA阳性率与前列腺癌骨转移及Gleason评分之间的关系。结果:PCa组尿液T1E4 mRNA含量(9.6×10-3,0~12.6)明显高于BPH组(0,0~8.5×10-3)(P<0.01)。尿液T1E4 mRNA含量诊断前列腺癌的曲线下面积(AUC)为0.810(95%CI:0.719~0.902),以1.0×10-5为截断值时,其诊断前列腺癌的敏感度和特异性分别为71.7%和81.8%。尿液T1E4 mRNA阳性率与前列腺癌骨转移及Gleason评分之间无关。结论:前列腺按摩后尿液中融合基因TMPRSS2:ERG亚型T1E4 mRNA含量检测对前列腺癌诊断有较高的敏感度和特异性,作为前列腺癌早期诊断指标具有良好的应用前景。 Objective:To investigate the utility of the quantitative detection of T1E4 mRNA,the isoform of TMPRSS2:ERG fusion gene in urine after prostate massage for the diagnosis of prostate cancer.Methods: T1E4 mRNA and PSA mRNA were measured by rea1-time fluorescent quantitative reverse transcription polymerase chain reaction(FQ-RT-PCR) based on Taqman technique in urine samples of 46 PCa and 44 BPH patients after prostate massage.PSA mRNA was used to normalize the number of prostate cells and T1E4 mRNA/PSA mRNA ratio was then used as a diagnostic marker for prostate cancer.The diagnostic efficacy of T1E4 mRNA in urine samples was evaluated by the ROC curve.Correlations of T1E4 mRNA with bone metastasis and Gleason score were analyzed.Results: The expression of T1E4 mRNA in PCa group(9.6×10-3,0~12.6) was significantly higher than that in the BPH group(0,0~8.5×10-3;P0.05).The AUC of T1E4 mRNA/PSA mRNA was 0.810(95%CI: 0.719~0.902).The diagnostic sensitivity and specificity of T1E4 mRNA/PSA mRNA were 71.7% and 81.8% respectively at the cut-off value of 1.0×10-5.The positive rate of urine T1E4 mRNA was not correlated with bone metastasis and Gleason score of PCa.Conclusion: The T1E4 mRNA in urine after prostate massage may be the potential marker which can be used in the early diagnosis of prostate cancer owing to its high sensitivity and good specificity.
