摘要
目的探讨防盲活动中贫困白内障手术质量控制方法。方法年终检查时(距手术后最短时间为一个月,大部分都在术后三月以上)设计了一套质量评估方案,即随机、按一定的比例进行检查,包括按8%抽查病历,2%电话随访,0.5%现场入户检查。结果 2009年我省贫困白内障手术任务13000例,实际完成13308例。手术方式主要为小切口非超声乳化白内障摘除及人工晶体植入术,其中8%的病人采取了超声乳化手术。病历抽查:93%的病历能按省卫生厅和省防盲办要求的规范执行,其中观察术后矫正视力>0.3达91.23%,人工晶体植入率97.82%。电话随访结果是老百姓对该项工程满意率达92.30%。现场入户调查结果,对手术效果(100分)评分98.87,对政府组织(100分)等评分99.98,现场视功能检查(包括日常生活对视力的需求)评分(总分40分)37.43。术后并发症发生率﹤5%,主要表现为瞳孔变形和后发白内障。结论按一定比例,随机采取病历检查、电话随访和现场入户相结合的方法进行调查,是贫困白内障患者复明工程手术质量的良好控制方法。
Objective Aim To explore the methods of quality control on cataract surgery in preventing blindness.Methods A plan was designed to evaluate the quality of cataract surgery at year-end check..Cases were checked randomly and by a certain percentage：medical records by 8%,phone call visit by 2%,on site visit by 0.5%.Results 13000 cases were planned to undergo free cataract surgery in Anhui and in fact 13308 cases underwent the surgery.Main operation method was small-incision non-phacoemulcification cataract surgery,while 8% cases underwent phacoemulcification.In check of medical records,93% of them were in accordance with the requests of Technique Guiding Center on Preventing Blindness of Anhui Province.91.23% of cases got a postoperative visual acuity better than 0.3.The implantation ratio of intraocular lens was 97.82.In phone call visit,92.3% of cases were satisfied with the surgical results.In on site visit,average score for surgical results was 98.87（maximal score is 100）,for government management was 99.98（maximal score is 100）,and for visual function in common life was 37.43（maximal score is 40）.Ratio of postoperative complications was less than 5%,including deformed pupil and after cataract.Conclusion It is a good quality control method to investigate the cataract surgery combined with medical record check,phone call visit and on site visit.
