肝硬化腹水并发自发性细菌性腹膜炎危险因素分析被引量：8

摘要目的探讨肝硬化腹水并发自发性细菌性腹膜炎(SBP)的易患因素,为临床早期诊断及时预防提供依据。方法将178列肝硬化腹水患者分为非SBP组和SBP组,将两组患者的临床资料进行组间比较,将有意义的单因素进一步行Logistic回归分析。结果单因素分析显示SBP病史、腹泻、上消化道出血、腹水总蛋白、血清总胆红素、血小板数、凝血酶原时间、C反应蛋白8个因素在两组间的差异有统计学意义(均P<0.05);而年龄、性别、肝硬化原因、血清白蛋白、有无腹腔穿刺、电解质紊乱、顽固性腹水、是否合并糖尿病、肝性脑病等因素的差异无统计学意义(均P>0.05)。Logistic回归分析表明:SBP病史、腹水总蛋白、血清总胆红素、C反应蛋白是肝硬化腹水并发SBP的独立危险因素。结论 SBP病史、腹水总蛋白、血清总胆红素和C反应蛋白4个因素是肝硬化腹水患者并发SBP的独立高危因素,对于具有这些高危因素的患者在临床中应加以重视。
