摘要
目的观察糖尿病患者在空腹血糖控制的不同状况下,应用尿微量白蛋白的定性检测方法对社区糖尿病肾病早期筛查的价值。方法将62例2型糖尿病患者按空腹血糖控制的结果分为三组,控制良好组、控制一般组、控制不良组,同时进行尿微量白蛋白的定性检测。结果尿微量白蛋白的定性检测,空腹血糖控制良好组阳性率11.76%(2/17),控制一般组阳性率28.57%(6/21),控制不良组阳性率45.83%(11/24),空腹血糖控制不良组的阳性率较前两组有显著差异(P<0.01),提示尿微量白蛋白与空腹血糖的控制不同水平呈正相关,应在检测空腹血糖时同时进行尿微量白蛋白的定性筛查。结论利用试纸进行尿微量白蛋白的定性检测,方法简单、快捷、经济,适应于社区医院具有糖尿病高危因素的人群和糖尿病肾病的早期筛查及发现,对早期干预和治疗有着重要的价值。
Objective To explore the predictive value of qualitative microalbuminuria detection in screening for diabetic nephropathy among diabetic patients with different fasting blood glucose levels. Methods A total of 62 type 2 diabetes patients were divided into well control group, normal control group and poor control group according to their fasting blood glucose levels,and they all had qualitative test for microalbuminuria. Results Qualitative microalbuminuria detection revealed positive rate of the poor control group was 45.83% （11/24）, significant different from the 11.76%（2/17）of the well control group and 28.57%（6/21）of the normal control group （P〈0.01）,indicating the positive correlation between the qualitative test result and the fasting blood glucose level. And it was shown qualitative microalbuminuria detection was a necessary complementation to fasting blood glucose monitoring. Conclusion With advantages of simple operation,quick result,and cheap price,qualitative microalbuminuria detection with Micral-test strips （Roche Diagnostics）is suitable for diabetic nephropathy screening among patients with high risks in commutity hospital since it shows great significance for the early intervention and treatment of this disease.
