摘要
【目的】观察调补肺肾法治疗特发性肺间质纤维化(IPF)的临床疗效。【方法】将42例特发性肺间质纤维化患者随机分为治疗组和对照组各21例,治疗组采用调补肺肾方(由补骨脂、山萸肉、五味子、丹参、紫苏子、黄精、淮山、浙贝母、枇杷叶、炙麻黄等组成)治疗,对照组采用强的松治疗。2组均以3个月为1个疗程。观察2组治疗后总体临床疗效、肺功能[肺总量(TLC)、肺活量(VC)、肺一氧化碳弥散量(DLCO)]及动脉血氧分压(PaO2)的变化。【结果】总有效率治疗组为66.7%,对照组为47.6%,2组比较,差异有显著性意义(P<0.05)。治疗后2组患者的TLC、VC、DLCO、PaO2水平均有明显改善(与治疗前比较,P<0.05),但2组间比较差异无显著性意义(P>0.05)。【结论】调补肺肾法既可改善IPF的症状,又可改善患者肺功能,从而进一步控制IPF的进展,提高患者生活质量,延长患者生存时间。
Objective To observe the therapeutic effect of therapy of tonifying lung and spleen for idiopathic pulmonary interstitial fibrosis （IPIF）. Methods Forty-two IPIF patients were equally randomized into two groups. The control group was given oral use of prednisone, and the treatment group was given oral use of decoction of Tiaobu Feishen Recipe （mainly composed of Fructus Psoraleae, Fructus Corni, Fructus Schisandrae Chinensis, Radix Salviae Mihiorrhizae, Fructus Perillae, Rhizoma Polygonati, Rhizoma Dioscoreae, Bulb Fritillariae Thunbergii, Folium Eriobotryae, honey-fried Herba Ephedrae, etc. ）. Three months constituted one treatment course. After one treatment course, the total therapeutic effect was evaluated, and the changes of arterial partial pressure of oxygen （PaO2） and pulmonary function such as total lung capacity （TLC）, vital capacity （VC） and diffusion capacity for carbon monoxide of lung （DLCO） were observed. Results The total effective rate was 66. 7% in the treatment group and 47.6% in the control group, the difference being significant （P 〈0.05）. After treatment, TLC, VC, DLCO and PaO2 were improved obviously in both groups （P 〈 0.05 compared with those before treatment ）, but the differences between the two groups were insignificant （ P 〉 0. 05 ）. Conclusion Therapy of tonifying lung and spleen can relieve the symptoms, improve lung function and the quality of life, prolong the survival time, and control the progress of idiopathic pulmonary interstitial fibrosis patients.
