摘要
目的探讨经脐单孔腹腔镜技术的可行性和临床应用前景。方法回顾性分析2009年4月～2010年4月新疆医科大学附属中医医院普外科完成58例经脐单孔腹腔镜手术患者的手术时间、出血量、并发症及术后恢复情况等。结果共完成56例经脐单孔腹腔镜手术,其中胆囊切除术30例,阑尾切除术18例,肝囊肿开窗术8例。2例经脐单孔腹腔镜胆囊切除术因操作困难中转为常规腹腔镜胆囊切除术。胆囊组手术时间为48～120min,平均(66±25)min;阑尾组手术时间为15～40min,平均(22±20)min;肝囊肿组手术时间为34～52min,平均(42±16)min。术中无明显出血。术后未发生胆漏、腹腔感染、切口感染等并发症。术后平均2.8d出院。手术切口瘢痕隐蔽不易察觉,病人对美容效果十分满意。结论经脐单孔腹腔镜手术可实现腹壁无瘢痕化,减少切口感染几率,但操作难度较大,可以在慎重选择病例的基础上开展。
Objective To evaluate the clinical feasibility of the transumbilical single-incision laparoscopic surgery.Methods The methods and clinical effects of 58cases of transumbilical single-incision laparoscopic surgery performed between April,2009and April 2010at department of general surgery affiliated to traditional Chinese medicine hospital of Xinjiang Medical University were analyzed.Results The transumbilical single-incision laparoscopic operations of 56cases were completed successfully while 2cases were converted to routine laparoscopic cholecystectomy due to difficulties in operations.There was no significant bleeding during the operation.The operating time of 30cases of laparoscopic cholecystectomy was 48 to 120minutes and the average surgery time was（66±25）minutes.The operating time of 18cases of laparoscopic appendectomy was 15to 40minutes and the average surgery time was（22±20）minutes.The operating time of 8cases of laparoscopic fenestration in hepatic cyst was 34to 52minutes and the average surgery time was（42±16）minutes.No postoperation complications such as biliary fistula,abdominal infection and wound infection occurred.Mean postoperative hospital stay was 2.8days.All cases showed good cosmetic outcomes.Conclusion The transumbilical laparoscopic surgery by single-incision is a new technique but a difficult procedure.The technique results in superior cosmesis.The feasibility of the clinical research is determined by the ability of the center,and patients should be selected carefully.
