无抽搐电休克治疗对精神障碍者脑电活动的影响及其相关因素分析 被引量:5

Effects of Modified Electric Convulsive Therapy on Brain Electrical Activity in the Patients with Mental Disorder and Its Analysis of Related Factors

摘要 目的:探讨无抽搐电休克治疗(MECT)对精神障碍者脑电活动的影响及其相关因素。方法:抽取经MECT(治疗次数≥6次)的住院精神障碍者(46例)的病案资料,分析抽搐发作时脑电图(EEG)各个成分波(δ、θ、α、β波)的幅度值和治疗后1周内的EEG,以及治疗前脑电活动、刺激电量、发作后抑制指数等相关因素。结果:MECT后的EEG异常发生率比治疗前高(P<0.01);Spearman相关分析显示MECT时δ、θ、α和β波的幅度值与抑制指数呈显著正相关(δ:P<0.05,θ、α、β:P<0.01),除β波外均与刺激电量呈显著负相关(δ:P<0.05,θ、α:P<0.01,β:P>0.05),而各个波的幅度值与年龄及治疗前、后脑电活动等因素的相关性分析无统计学意义。结论:MECT对精神障碍者脑电活动有一定的影响;治疗发作时δ、θ、α和β波的幅度值与发作后抑制指数及刺激电量关系密切。 Objective: To investigate the effects of modified electric convulsive therapy( MECT) on brain electrical activity and its related factors in the patients with mental disorder. Methods: The clinical data of 46 inpatients(more than 6 times by MECT)with mental disorder were selected. The amplitude values of each waves(δ、θ、α、β)for EEG during the induced seizure and EEG within a week after MECT and the factors in brain electrical activity before treatment, stimulating electric quantity, postictal suppression indexes were analyzed. Results: The abnormal incidence for EEG after MECT was higher than that before( P 〈 0.01). Spearman correlation analysis implied that there was a positive correlation between the amplitude values(δ、θ、α、β)and postictal suppression indexes during MECT(B P 〈 0.05, θ、α、βP 〈 0.01), there was a negative correlation between the amplitude values(δ、θ、α)and stimulating electric quantity(δ P 〈 0.05, θ、α P 〈 0.01). There was no statistical significance between the amplitude values of each waves and other factors in age, electrical activity of brain before and after MECT. Conclusion: There must be some influences to the brain electrical activity in the patients with mental disorder. The amplitude values(δ、θ、α、β)during seizure have intimate con'elation with postictal suppression indexes and stimulating electric quantity.
