摘要 目的:探讨CT扫描对颈部神经源肿瘤的定位、定性诊断价值。材料与方法:复习经手术及病理证实的颈部神经源肿瘤的CT扫描30例,包括神经鞘瘤17例,神经纤维瘤6例,颈动脉体瘤3例,恶性神经源肿瘤4例。结果:肿瘤位于颈动脉间隙15例,椎旁间隙9例,其他部位6例。颈动脉间隙肿物中,8例(53.3%)使颈动、静脉向外侧移位,2例(13.3%)使血管向前方移位,5例(33.3%)使颈动、静脉分离(其中4例明确来源于迷走神经)。9例椎旁间隙肿物均使前斜角肌向前及外侧移位,2例有椎间孔扩大。17例神经鞘瘤中肿瘤密度低于颈部肌肉15例(88.2%),肿瘤呈低密度环包绕中央的网状、云雾状或岛状高密度区(35.3%)或相反(23.5%)或呈混杂密度(41.2%)。结论:增强CT扫描极有助于颈部神经源肿瘤的定位及定性诊断,最有价值的诊断指征为肿瘤的部位、肿瘤与邻近结构的关系以及肿瘤的密度变化。 Objective: To study the ability of CT for the localization and characterization of cervical neurogenic tumor. Materials and Methods: CT scans of 30 cases of the cervical neurogenic tumor proved by surgery and pathology were reviewed, which included schwannoma ( n= 17), neurofibroma ( n= 6), carotid body tumor ( n= 3) and malignant neurogenic tumor ( n= 4). Results: The location of the tumors were: carotid space (WT5BXn= 15, 16 lesions), paraspinal space ( n= 9), miscellaneous ( n= 6). Of the tumors located in the carotid space, there were lateral displacement of the carotid artery (CA) and internal jugular vein (IJV) in 8 cases (53.3%), anterior displacement of the vessels in 2 cases (13.3%). The CA and IJV were separated apart in 5 cases (33.3%), of which, 4 tumors originated from vagus nerve. Anteriorand lateral displacement of the anterior scalenus muscle were shown in all of the 9 tumors located in the paravertebral space, enlargement of the vertebral foramen was found in 2 cases. 88.2% of the 17 cases of the schwannoma had the density lower than that of cervical muscles. All of the schwannomas were heterogeneous in density, which were central insular speckled high attenuation area surrounded by a low attenuation rim (35.3%) or vice versa (23.5%), salt and pepper alike mixed attenuation (41.2%). Conclusion: Contrast enhanced CT is very helpful in the diagnosis of cervical neurogenic tumor, the most valuable diagnostic parameters included location of the tumor, relationship between the tumor and the adjacent great vessels as well as the density of the tumor.
