摘要
目的:探讨全膀胱切除回肠原位新膀胱术患者围术期的护理方法。方法:对31例全膀胱切除回肠原位新膀胱术患者做好术前、术后护理。结果:本组手术均顺利进行,手术时间240～360 min,平均260 min;术中输血0～800ml,平均400 ml。术后随访3个月患者均排尿通畅,每夜尿2～3次;6个月肠黏液明显减少,所有病例初期均有残余尿;12个月残余尿均能控制在50 ml以下,可完全恢复日间尿控,其中3例患者出现轻度夜间尿失禁。结论:加强全膀胱切除回肠原位新膀胱术患者围术期护理,可提高手术成功率,减少并发症发生。
Objective： To explore the nursing methods in perioperative period of total cystectomy and orthotopic ileal neobladder.Methods： 31 patients treated with total cystectomy and orthotopic ileal neobladder were given careful preoperative and postoperative nursing care.Results： The operations were performed smoothly.The operation time was 240 ～ 360 minutes and the average time was 260 minutes;blood transfusion in the operation was 0 ～ 800 ml and the average was 400 ml.The patients had free urination by follow-up for 3 months and they urinated 2 to 3 times at night;intestinal mucus significantly reduced 6 months later,all patients had residual urine in the initial period;the residual urine can be controlled below 50 ml after 12 months and daytime control of urination restored completely.The mild incontinence at night occurred in 3 patients.Conclusions： Strengthening perioperative nursing care in total cystectomy and orthotopic ileal neobladder can improve the success rate of the surgery and reduce the incidence of complications.
