摘要
目的探讨纤维鼻咽喉镜在鼻内镜术后上颌窦术腔病灶处理的临床应用价值。方法联合应用纤维鼻咽喉镜技术处理鼻内镜手术后上颌窦术腔病灶46例,并针对不同的病变采用不同方法处理。结果随访6个月～1.8年,平均9.3个月。治疗前后的视觉模拟量表(visual analogue scale,VAS)主观评价轻、中、重度病例数发生明显改变,分别由术前的15.22%、67.39%、17.39%改变为术后的71.74%、28.26%、0%,其中提示生活质量受影响的VAS>5由65.22%改变为10.87%,治疗前后Lund-Kennedy的客观评分中,息肉、水肿、鼻漏、瘢痕、结痂评分及总分均有明显减少,差异均有统计学意义(P<0.05)。结论术后联合应用纤维鼻咽喉镜能有效处理上颌窦术腔残留或复发的病灶,提高上颌窦手术效果。
Objective To evaluate a value of fibrolaryngoscope in treatment the focus of infection in postoperative maxillary sinus cavities. Methods Nasal endoscope and fibrolaryngoscope were used jointly in the treatments on 46 cases with focus of infection in postoperative maxillary sinus cavities after nasal sinus surgery, and the patients were performed with different methods according the pathological changes. Results After fellow-up for 6 months to 1.8years, the subjective symptom scores with VAS were all decreased after the treatment, and the difference was statistically significant, from 15.22% to 71.74, from 67.39% to 28.26% and 17.39% to 0, respectively. Some of them with VAS〉5 decreased from 65.22%to 10.87%. A 11 of Lund-Kennedy scores decreased（P〈0.05）.Conclusion The fibrolaryngoscope is greatly effective in treatment the residual and recurrent affections of the postoperative maxillary sinus cavities, especially at clearing those in the maxillary anteriorwall, anteriorwall, lateralwall and the bottom wall, which can improve the cure rate of endoscopic maxillary sinus surgery.
