摘要
目的:研究原发性慢性闭角型青光眼(primary chronic angle-closure glaucoma,PCACG)发病机制中晶体因素的影响。方法:采用A/B超、超声生物显微镜等对原发性慢性闭角型青光眼25例42眼及正常眼24例45眼的晶体参数进行测量和计算。比较组间晶体参数的差异。结果:原发性慢性闭角型青光眼与正常组比较,晶体中央及周边部厚度、前、后表面曲率半径、相对位置、晶体体积、虹膜晶体接触距离,有统计学差异(P<0.05);PCACG组的晶体直径与正常组比较,无统计学差异。且随年龄增加,前房深度、晶体体积、晶体前、后表面曲率均有增加。结论:晶体因素导致的瞳孔阻滞是原发性慢性闭角型青光眼发病的主要诱因,包括晶体较厚,前、后表面曲率较大和相对位置偏前、虹膜晶体接触距离较大等。
Objective:To study the role of lens disorder in the pathogenesis of primary chronic angle-closure glaucoma(PCACG).Methods:The lens parameters were measured and calculated by A/B ultrasound and ultrasound bio-microscope for 42 eyes in 25 PCACG patients(PCACG group)and 45 eyes in 24 healthy individuals(control group).The lens parameters were compared between the two groups.Results:There were significant differences in the central and peripheral lens thickness,anterior and posterior surface radius of curvature,relative position,lens size and iris lens contact distance between the two groups(P 0.05).There was no significant difference in the lens diameter between the two groups.With increase of age,the anterior chamber depth,lens size and anterior and posterior surface radius of curvature were increased.Conclusion:The pupil block induced by lens disorder is the primary cause of PCACG,including thicken of lens,increase of anterior and posterior surface curvature,antedisplacement of relative lens position,enlargement of the iris lens contact distance and so on.
