脑卒中患者内皮源性血管活性物质与局部脑血流量的关系 被引量:13

The relationship between the endothelium derived vasoactive factors and regional cerebral blood flow in acute stroke

摘要 目的探讨急性脑卒中患者血浆内皮源性血管活性物质内皮素(ET)、血栓素B2(TXB2)、6酮前列腺素F1α(6KPGF1α)浓度变化及其与局部脑血流量(rCBF)的关系。方法应用放射免疫分析法测定64例脑卒中患者和30例正常对照者的血浆ET、TXB2、6KPGF1α浓度,并同步应用133Xe吸入法测定rCBF。结果脑卒中患者血浆ET含量为(1038±426)ng/L、TXB2为(1522±591)ng/L,正常对照组分别为(478±78)ng/L和(848±115)ng/L,两者相比,差异均有显著性(P均<001);而6KPGF1α含量为(937±288)ng/L,较正常对照组的(1047±174)ng/L显著下降(P<005)。脑卒中患者左、右半球和平均rCBF分别为(466±88)、(472±82)和(469±79)mL·100g-1·min-1,明显低于正常对照组的(622±71)、(645±71)和(633±65)mL·100g-1·min-1,P<001。血浆ET、TXB2升高和rCBF下降在大面积卒中较小面积卒中明显,基底节卒中? Objective To explore the plasma concentration of the endothelium derived vasoactive factors such as endothelin (ET), thromboxane B 2 (TXB 2), 6 keto prostaglandin F 1α (6 K PGF 1α ) and their effects on regional cerebral blood flow (rCBF) Methods Plasma ET, TXB 2, 6 K PGF 1α were measured with radioimmunoassay in 64 patients with acute stroke and 30 control subjects Meanwhile, the rCBF was determined using 133 Xe inhalation method in all patients and the control group The data of stroke group were studied by t test The linear correlation between alterations of vasoactive factors and rCBF was analysed Results The mean ET, TXB 2 plasma level [respectively (103 8±42 6) and (152 2±59 1) ng/L] was significantly higher in 64 stroke patients than in normal subjects [respectively (47 8±7 8) and (84 8±11 5) ng/L], P<0 01 The patients had lower plasma 6 K PGF 1α [(93 7±28 8) ng/L] as compared with the healthy controls [(104 7±17 4) ng/L], P<0 05 Meantime, rCBF in patients was significantly lower than that in healthy age matched controls [left: (46 6±8 8) mL·100 g -1 ·min -1 vs (62 2±7 1)mL·100 g -1 ·min -1 ; right: (47 2±8 2) mL·100 g -1 ·min -1 vs (64 5±7 1) mL·100 g -1 ·min -1 ; mean: (46 9±7 9) mL·100 g -1 ·min -1 vs (63 3±6 5) mL·100 g -1 ·min -1 , P<0 01] Moreover, plasma ET, TXB 2 in patients with large infarct or hemorrhage volume were markedly higher than those of patients with small foci; to the opposite, rCBF was decreased remarkably The same situation was seen while compared the date of patients with basilar nuclei stroke with those of patients with lobar stroke Both ET and TXB 2 had a significant negative correlation with rCBF (r=-0 751, -0 454, P<0 01) Bilateral rCBF decrease was seen in 43 patients with unilateral stroke Conclusions This study suggests that there was endothelium dysfunction in the cerebral arteries of patients with acute stroke Plasma level of ET, TXB 2 and rCBF might be useful in assessment of brain damage caused by acute stroke
