摘要
目的 :为了缩短麻醉手术时间 ,对联合阻滞与连续硬膜外麻醉方法进行比较 ,选其最佳的麻醉方法 ,提高妇产科麻醉水平。方法 :选连续硬膜外妇产科手术 2 0 0例(剖宫产 10 0例 ,子宫切除术 10 0例 ) ;联合阻滞麻醉 2 0 0例 (剖宫产 10 0例 ,子宫切除术10 0例 )。局麻药均用 2 %利多卡因。结果 :两组手术时间不变 (P >0 0 5),麻醉时间明显缩短 ,其主要原因是由于联合阻滞麻醉诱导时间缩短 ,局麻药的用药剂量也明显减少。而循环及辅助用药情况 ,联合阻滞麻醉下行子宫切除术优于连续硬膜外麻醉。剖宫产组 ,虽其联合阻滞麻醉用药量小、阻滞完善 ,但血压下降机率大于连续硬膜外组。结论 :联合阻滞麻醉应用于妇产科手术 ,能大大缩短麻醉诱导时间 ,因而也就减少整个麻醉手术时间。同时 ,麻醉作用完善 ,对循环影响低于硬膜外麻醉组 ,故此麻醉方法值得推广。联合阻滞剖宫产组 ,对妊娠合并高血压者 ,应密切注意循环变化。
Objective:In order to reduce the duration of surgery and anesthesia and enhance the level of anesthesia in Obstetrics and Gynecology,we compared combined spinal epidual anesthesia(CSEA) and epidual anethesia(EA) and chose the best technique in Obstertrics and Gynecology anesthesia Methods:200 parturients who undergoing cesarean in intrathecal anesthesia were randomly divided into two groups CSEA groups(n=100) and EA groups(n=100),and so were 200 femaie parturient who undergoing hysterectomy.2% lidocaine was administered as a local anesthetic drug.Results:Compared with no difference between the duration of same surgery(P>0.05),the period of anesthesia was shorter in CSEA groups than in EA groups significantly.The main reason of it is that CSEA reduce the time of induction,on the other hand,the total dose of the local anesthesia drugs decreased greatly.As to hemodynamic and adjuvant,CSEA was better than EA in hysterotomy patients.To parturients,less local anesthetic drug and more adequent blockade,CSEA had higher incidence and frequency of hypotension.Conclusion:Applied for Obstetric and Gynecology surgery,CSEA reduce the period of induction of anesthesia,which shortens the total duration of surgery.At the same time,more adequent anesthesia and more stable hemodynamic,CSEA are worth using widely.When used CSEA parturient with hypertension should be closely observed blood pressure.
