摘要
为探讨心电图与心脏形态及功能的关系,用二维、多普勒超声心动描记术测定110例PTFv_1负值增大患者的心脏形态结构及心功能参数,并以56例PTFv_1值正常者为对照.结果显示:(1)PTFv_1异常组LAD、LVD(分别为38.84±6.34及56.54±10.94mm)较正常组(分别为30.73±3.75及48.28±6.26mm)明显增大(P<0.01);RVD、RVD、IVST及PWT两组无明显差别(P>0.05).(2)PTFv_1异常组EPFV(72.75±28.41cm/s)校对照组(81.31±24.35cm/s)降低(P<0.05),APFV(78.08±25.90cm/s)较对照组(66.84±22.21cm/s)明显增高(P<0.01),E/A明显减低(分别为0.92±0.39及1.43±0.82,P<0.01,E/A<1.两组SV、CO、CI、及LAEF差异无显著意义(P>0.05).(3)PTFv_1异常组,PTFv_1值与LAD及LVD均呈显著负相关(r=-0.66、-0.31,P<0.01).提示:(1)PTFv_1值可作为左心结构形态,尤其是左心房内径的判定指标.(2)PTFv_1负值增大可作为判断左心室舒张期负荷增重、早期诊断左心室功能障碍的辅助指标.
To study relationship between electrocardiogram and heart morphology or (unction, parameters of heart morphology and structure its function were measured by echocardiography in 110 patients with ab-normal PTFv1 - 0.04mm ?s) and 56 patients with normal PTFv1 ( > - 0.02mm ?s) . Results revealed: (1) there is a larger LAD and LVD (38.84?6.34mm and56.54?10.94mm) in the patients with abnormal PTFv1 than patients with normal PTFv1 (30.73 ?3.75mm and 48.28 ?6.26mm, P < 0.01); RVD, IVST and PWT have no significant diference between two groups(P > 0.05); (2) comparing with control group(EPFV = 81.31 ?24.35cm/ s, APFV = 66.84 ?22.21cm/s,E/A> 1) ,EPFV(72.75 ?28.41cm/s) is lower, APFV (78.08 ?25.90cm/s) is higher and E/A < lin abnormal PTFv1 group (P < 0.01); while there is no significant difference on SV, CO, CI and LVEF between two groups (P > 0.05); (3) in abnormal PTFv1 group, there is a significant negative correlation betweem PTFv1 and LAD or LVD (r= -0.66 and r= - 0.31 respectively, P <0.01) . It is concluded that (1) PTFv1 may be one of standards for heart morphology LAD and LVD, especially LAD; (2) abnormal PTFv1 may reflect a decreased diastolic function of left ventrical; PTFv1 may be one of standards reflecting left ventrical diastolic load; it may be used to make an early diagnosis of left ventrical dysfunction.
