摘要
目的通过对综合医院一名神经科总住院医师的945例会诊病例分析,反映总住院医师的工作情况。方法统计分析945例提出神经内科会诊的非神经内科住院患者和急诊室患者会诊情况。结果共新会诊患者945例,女494例(52.3%),男451例(47.4%),年龄分布在15～94岁之间,平均每周新会诊39例。急诊室会诊419例(44.3%),病房会诊526例(55.7%),包括病房常规会诊432例(82.2%),病房急症会诊94例(17.8%)。内科约请会诊以脑血管病居多,86例(27.1%),其次为代谢性脑病40例(12.6%),会诊疾病出现频率较高的还有无明确器质性病因的头迷或眩晕38例(12%);头痛32例(10.1%);周围神经病,28例(8.8%)。外科和妇产科约请会诊目的主要是术前准备、围手术期神经科疾病风险评价和预防,32例(28.7%)。肿瘤科会诊病例则以中枢神经系统转移性肿瘤居多,30例(36%)。结论总住院医师制度,使总住院医师获得了更多的临床实践机会。神经科医生需要在神经病学、内科学及心理学方面具有广博的知识。
【Objective】 To describe the profile of a neurology chief resident in China.【Methods】 The pattern and frequency of patient encounters during the Shengjing Hospital Affiliated to China Medical Univer-sity neurology residency program for one chief resident were retrospective analyzed.【Results】A total of 945 new inpatients were seen during a 6-month period（39 new patients per week）.This total represented 419 e-mergency outpatients（44.3%） and 526 inpatients（55.7%）.For routine consultation on general internal medicine,vascular disease（n =86） was the most common one.For consultation on surgery,most cases were concerned about preoperative evaluations of the neurologic diseases（n =32）.While on oncology,those were brain/spinal metastasis（n =30）.【Conclusion】 The chief resident would encountered more patients.Neurologists require broad-based training in neurology,internal medicine,and psychiatry,to ensure excellence in clinical practice.
