实时三维超声心动图评价法洛四联症主动脉骑跨 被引量:2

Assessment of overriding aorta of tetralogy of Fallot with real-time three-dimensional echocardiography

摘要 目的评价实时三维超声心动图(RT-3DE)评估法洛四联症(TOF)主动脉骑跨的价值。方法 TOF患儿25例,选取25名同年龄组健康儿童作为正常对照组,测量两组体质量、身高,测量TOF患儿主动脉骑跨的二维超声测值(骑跨率)和三维超声测值(骑跨面积比),与手术结果进行比较,并对骑跨率与手术结果行Pearson相关分析。结果 TOF患儿的身高、体质量均小于正常对照组(P均<0.01)。二维超声所测骑跨率为0.35±0.31,与手术结果0.45±0.08的差异有统计学意义(P<0.01),但与手术结果无相关关系(r=3.15×10-4,P>0.01)。三维超声所测骑跨面积比为(0.47±0.11),与手术结果(0.45±0.08)的差异无统计学意义(P>0.01)。结论三维超声测量的TOF患儿骑跨面积比与手术结果无明显差异;三维超声评价主动脉骑跨比二维超声更加准确。 Objective To assess the value of real-time three dimensional echocardiography (RT-3DE) for evaluating the overriding aorta in tetralogy of Fallot (TOF). Methods Twenty-five children with TOF and 25 healthy children of the same age were enrolled,and their height and weight were measured. The overriding degree of aortic root was measured with two-dimensional echocardiography (overriding rate) and RT-3DE (overriding area),both were compared with operation results. The correlation between overriding ratio of aortic root and operative results was analyzed with Pearson correlative test. Results The weight and height of TOF children were lower than those of healthy children (all P0.01). The overriding rate was 0.35±0.31,statistically different with operation results (0.45±0.08,P0.01).No correlation was found between these two methods (r=3.15×10-4,P0.01).The overriding area ratio was 0.47±0.11,not statistically different with operation results (0.45±0.08,P0.01). Conclusion There is no significant difference between the overriding area ratio of aortic root and the operative results. RT-3DE is more accurate than two-dimensional echocardiography in assessing overriding degree of aortic root.
