冠心病患者体脂分布与冠状动脉病变程度的关系 被引量:8

Relationship between body fat composition and the severity of coronary artery disease

摘要 目的:了解冠心病患者体脂分布与冠状动脉(冠脉)病变程度的关系。方法:69例冠心病患者,用MSCT评估心外膜脂肪体积(EFV)、腹部总脂肪体积(TFV)、内脏脂肪体积(VFV)、皮下脂肪体积(SFV),同时测量身高、体重,计算体质指数(BMI),测量腰围、臀围,计算其比值(WHR),分别通过Gensini积分系统和冠脉粥样斑块积分系统评估冠脉病变程度。分析体脂分布与冠脉病变程度的关系及EFV与各种体脂评价指标的关系。结果:体重、BMI、腰围、WHR、VFV、TFV、EFV与冠脉病变评分呈正相关(P<0.05),以VFV和EFV与冠脉病变相关性最强(r=0.356)。EFV与体重、BMI、腰围、臀围、WHR、VFV、TFV呈正相关相关(P<0.05),与VFV相关性最强(r=0.630),与SFV无明显相关。结论:在冠心病患者中,VFV及EFV是反映冠脉病变程度的较佳指标,VFV与EFV具有较好的相关性,可以通过心外膜脂肪的量评估内脏型肥胖。 Objective:To explore the relationship between body fat distribution and the severity of coronary artery disease and compare the various methods of evaluating body fat distribution.Method:Sixty-nine coronary heart disease patients were enrolled.Epicardial fat volume(EFV),abdominal visceral fat volume(VFV),total fat volume(TFV)and subcutaneous fat volume(SFV)were measured by MSCT.Body length and body weight were measured,and to compute body mass index(BMI).Waist circumference(WC)and hip circumference(HC)were measured,and to compute waist to hip ratio(WHR).The severity of coronary artery disease was assessed by evaluating the extension of coronary plaques and the percentage coronary diameter stenosis.Analyze the relationship between body fat distribution and the severity of coronary artery disease,EFV and other indexes.Result:A positive correlation was shown between body weight,BMI,WC,WHR,VFV,TFV,EFV and the severity of coronary artery disease(P0.05).The relationship of VFV and EFV with the severity of coronary artery disease is the closest(r=0.356).A positive correlation was shown between body weight,BMI,WC,HC,WHR,VFV,TFV and EFV(P0.05).The relationship of EFV and VFV is the closest(r=0.630).Conclusion:VFV and EFV have a better relationship and could be a marker to reflect the severity of coronary artery disease.Visceral fat could be assessed by evaluating Epicardial fat.
