摘要
目的 介绍一种应用前臂预制耳郭游离移植进行耳再造的方法.方法 Ⅰ期手术,将自体肋软骨雕刻成耳支架后移植于前臂桡侧皮瓣区域,预制皮下血管化耳郭;Ⅱ期手术,切取携带预制耳郭的前臂皮瓣,经过血管吻合移植于耳缺损区,实现耳郭再造.自1999年7月至2009年6月,对3例患者行耳再造术.结果 术后随访3例患者6～15个月,1例患者因耳胆脂瘤感染致使再造耳外形不佳,2例患者效果满意.结论 前臂预制耳郭游离移植耳再造术对于乳突区缺乏正常皮肤和伴有面部发育不良的患者是一种较好的选择.
Objective To introduce a method of ear reconstruction by auricle free grafting with a prefabricated free forearm flap. Methods At the first stage, an ear framework carved from autogenous costal cartilage was transferred into subcutaneous stratum at forearm to prefabricate vascularize auricle; at the second stage, the forearm flap with cartilage frame was harvested and transplanted to the ear defect by vascular anastomosis. From July 1999 to June 2009, 3 patients underwent the operation. Results After 6 - 15 months follow- up, 2 cases received satisfactory results but 1 case was unsatisfactory of the appearance due to infection. Conclusion Ear reconstruction by auricle prefabricating at forearm is a good choice for patients who lost normal post auricular skin and those who suffer from facial dysplasia.
