深圳市妇女不同年龄段人乳头瘤病毒感染的流行病学分析 被引量:4

摘要 目的在深圳市通过调查妇女中高危型人乳头瘤病毒(Human,Papillomavirus HPV)感染状况,探讨参加过和从未参加过子宫颈癌筛查妇女中HPV感染的年龄分布情况差异,对持续高感染状态年龄阶段的深圳妇女进行子宫颈癌的防治提供科学依据和人群基础。方法 2006年~2008年在深圳以20~60岁有性生活妇女为对象,一组是深圳市福田区每年参加子宫颈癌筛查的女教职工随机选择2052人,第二组是深圳市龙岗区紫薇社区未曾参加过子宫颈癌筛查的妇女随机选择764人,进行HPV感染状况流行病学调查。进行以人群为基础的调查研究。对所有调查妇女子宫颈脱落细胞采用第二代杂交捕获技术(Hybrid Capture2,HC2)进行高危型HPV DNA检测。标本中检出的HPV DNA RLU/CO≥1.0pg/ml即为感染阳性。应用SPSS软件进行统计分析。结果共调查第一组福田区参加过筛查女教职工2502人,高危型HPV感染阳性者247人,感染率12.00%,共调查第二组龙岗区紫薇社区未参加过筛查妇女764人,高危型HPV感染阳性者122人,感染率16.00%,第二组未参加过筛查妇女高危型HPV感染阳性率高于第一组参加过筛查,两者比较有显著性差异(P<0.05)。第一组,不同年龄段中(20~,30~,35~,40~,45~,50~60岁)的HPV感染率分别为13.4%,10.60%,12.60%,14.70%,9.73%,6.06%),第二组不同年龄段中(30~,35~,40~,45~,50~60岁)的HPV感染率分别为16.90%,12.20%,19.40%,14.60%,17.30%。两组中40~44岁妇女中,HPV感染率均明显高于同组中其他年龄段(P<0.05)有显著性差异。20~30岁年龄组的感染率为13.40%,也是感染高峰,但是暂时性的,两组除35~39岁年龄段外,其余各年龄段比较第二组均明显高于第一组,20~30,30~40,50~60岁年龄段两组间比较有显著性差异(P<0.05)。结论这是深圳市规模较大的调查,人群顺从性较高,其结果能代表深圳参加过子宫颈癌筛查和未参加过筛查妇女的高危型HPV感染率的差别及其感染的年龄分布状况。40~44岁妇女是呈持续高感染状态,对这一年龄阶段的妇女进行计划的子宫颈癌的防治成为重点。为深圳子宫颈癌的防治提供流行病学数据。
