摘要
目的评价达英-35联合二甲双胍治疗肥胖型多囊卵巢综合征(PCOS)的疗效。方法选取59例肥胖型PCOS患者,分为对照组(予达英-35治疗)和治疗组(予达英-35联合二甲双胍治疗),治疗3个月。治疗前后测定体质量指数(BMI)、腰臀比(WHR)、促黄体生成素(LH)、睾酮(T)和性激素结合球蛋白(SHBG),并进行口服糖耐量试验(OGTT)和胰岛素释放试验,计算游离雄激素指数(FAI)、葡萄糖和胰岛素曲线下面积、空腹血糖/胰岛素之比(GIR)、OGTT各时点的胰岛素/血糖比以及卵巢体积。结果 2组治疗前后BMI、WHR和FSH无改变,2组LH、LH/FSH值、FAI和卵巢体积均较治疗前明显下降(P均<0.01),T也较治疗前降低(P<0.05),SHBG明显升高(P<0.01),但治疗组FAI水平下降较对照组下降明显(P<0.05);2组治疗前后空腹血糖均在正常范围,葡萄糖曲线下面积(AUC(血糖))无改变,治疗后胰岛素曲线下面积(AUC胰岛素)、OGTT各时相胰岛素/血糖比和胰岛素水平均升高(P均<0.01)。对照组GIR较治疗前显著下降(P<0.01),而治疗组GIR较治疗前显著升高(P<0.01),治疗组较对照组GIR明显升高。结论达因-35与二甲双胍联合运用能有效改善肥胖型PCOS患者临床症状,降低胰岛素抵抗,恢复排卵及月经周期。
Objective It is to evaluate the curative effect of Diane-35 combined with metformin on obese polycystic ovary syndrome（PCOS）.Methods 59 patients with obese PCOS were divided into two groups.30 cases were in control group and treated with Diane-35.The other 29 cases were in treatment group and treated with Diane-35 combined with metformin.The course of treatment in the two groups was both three months.Before and after treatment,body mass index（BMI）,waist to hip ratio（WHR）,luteinizing hormone（LH）,testosterone（T） and sex hormone-binding globulin（SHBG） were determined,oral glucose tolerance test（OGTT） and insulin releasing test were done and free androgen index（FAI）,area under curve of glucose and insulin,fasting blood glucose to insulin ratio（GIR）,insulin to blood glucose ratio at all the time of OGTT and ovary volume were computed.Results BMI,WHR and FSH in the two groups before and after treatment remained unaltered.LH,LH/FSH,FAI and ovary volume in the two groups after treatment were obviously lowered（all P 0.01）,T in the two groups after treatment was lowered（P 0.05） and SHBG in the two groups after treatment was obviously heightened（P 0.01）.The level of FAI in treatment group was decreased obviously than that in control group（P 0.05）.Fasting blood glucose and area under curve of glucose in the two groups had no changes.Area under curve of insulin,insulin to blood glucose ratio at all the time of OGTT and the level of insulin in the two groups were higher than that before treatment（all P 0.01）.GIR in control group after treatment was significantly lowered than that before treatment（P 0.01）,but GIR in treatment group after treatment was significantly heightened than that before treatment（P 0.01）.GIR in treatment group was significantly higher than that in control group.Conclusion Diane-35 combined with metformin can effectively improve the clinical symptom of obese PCOS,lower insulin resistance and recover ovulation and menstrual cycle.
