摘要
目的探讨腹腔镜下子宫动脉阻断后行子宫肌瘤剥除术的临床疗效及可行性。方法对2006年4月～2009年10月我院收治的因子宫肌瘤行腹腔镜下肌瘤剥除术的87例患者进行回顾性分析,其中32例行腹腔镜下子宫动脉阻断后肌瘤剥除术(研究组),55例行单纯腹腔镜下子宫肌瘤剥除术(对照组),观察两组的手术时间、术中出血量、肛门排气时间、术后病率等。结果研究组术中出血量(76±26.6)ml低于对照组(238±65.4)ml(P<0.05),术后病率研究组(9.4%)低于对照组(32.7%)(P<0.05)。结论腹腔镜下子宫动脉阻断后行肌瘤剥除术,大大减少了术中出血量,降低了术后病率,适当扩大了手术指征,有较好的临床应用价值。
Objective To evaluate the clinical feasibility and therapeutic effect of laparoscopic unterine artery occlusion im myomectomy.Methods From April 2006 to October 2009,87 patients with fibroid performed by laparoscopic myomectomy were retrospective analyzed.32 cases were performed laparoscopic uterine artery occlusion in myomectomy while the other 55 cases as control group were performed laparoscopic myomectomy only.The time of operation,intraoperative blood loss and postoperative data were evaluated and compared.Results Intraoperative blood loss of study group was less than control group[（76±26.6）ml vs（238±65.4）ml,P0.05];the febrile morbidity of study group was lower than control group（9.4% vs 32.7%,P0.05）.Conclusion Compared to conventional mothod,laparoscopic uterine artery occlusion in myomectomy presents less blood loss,lower febrile morbidity and may expand surgical indications of laparoscopic myoectomy.
