摘要
目的利用角膜共焦显微镜对配戴角膜塑形镜的角膜组织形态进行分析,为临床合理配戴角膜塑形镜提供客观依据。方法佩戴角膜塑形镜的患者40例(40只眼),佩戴框架眼镜且从未佩戴过隐形眼镜者10例做为对照。对照组为G1;G2为佩戴角膜塑形镜的时间长度为6个月至1年以内者;G3为1年至3年以内者;G4为3年至5年以内者;5年以上者为G5,每一组均为10例20只眼。采用Confoscan-4共聚焦显微镜扫描全角膜,记录角膜中央及角膜边缘Langerhans细胞、角膜上皮层、中央角膜厚度、角膜基质细胞及角膜内皮数。采用SPSS进行统计学分析。结果与对照组相比,随佩戴时间的延长,四组患者的角膜中央及角膜边缘Langerhans细胞的细胞密度均逐渐增多;角膜上皮层逐渐变薄;角膜全层厚度逐渐下降;后基质细胞密度降低;角膜内皮细胞数量无改变,但角膜多形性内皮细胞百分比逐渐降低,而异型性内皮细胞百分比逐渐升高。结论长期配戴角膜塑形镜所引起的角膜慢性缺氧可能是导致上述的角膜各层变化的主要原因,但5年后基本可以达到一个动态平衡的状态,这为指导患者用眼以及临床医师如何合理应用角膜塑形镜提供一个科学有效的依据。
Objective Observing the corneal morphous by the confocal microscopy to direct the clinic.Methods There are 40 cases who used corneal moulding in experiment groups,as G2,G3,G4 and G5 and 10 cases who haven't in control group,as G1.We used Confoscan-4 confocal microscopy to scan cornea,then recorded the Langerhans cell in central and margin of cornea,corneal epithelium,the deep of corneal central,corneal stroma cell and corneal endothelium.After this we used SPSS to statistics.Results Compared with the control,the long who used the corneal moulding the the Langerhans cell in central and margin of cornea is growing in number,the corneal epithelium is thinning,the deep of cornea is falling off and the stroma cell density is lossing,but the population of corneal endothelium is no change except of the percentage of the corneal pantomorphia cell is cutting down and the heteromorphism cell is stepping up.Conclusion The variance of the corneal morphous is resultsed from the hypoxia because of the corneal moulding.But after five years all variance will be mobile equilibrium and these are useful for clinic.
