摘要
目的探讨超声造影对脂肪肝伴实性占位病变的良恶性诊断和鉴别诊断的临床价值。方法对经病理证实的脂肪肝伴实性占位70例(91个病灶)采用超声造影技术实时观察,并与增强CT进行对照分析。结果超声造影和增强CT诊断敏感性、特异性、阳性预测值、阴性预测值、准确性分别为97.2%、92.1%、94.5%、96.1%、96.7%和93.4%、94.1%、95.1%、90.8%、94.1%,两者具有较好的一致性,与病理诊断均具有良好的信度(Kappa值分别为0.92、0.86)。结论超声造影对诊断和鉴别诊断脂肪肝背景下实性占位具有重要的临床价值。
Objective To investigate the clinical value of contrast-enhanced ultrasound（CEUS） in the differential diagnosis of focal liver lesions in fatty liver patients.Methods Ninety-one lesions in 70 patients with fatty liver confirmed by pathology were observed using real-time contrast-enhanced ultrasound and analyzed in lesions with contrast-enhanced CT.Results The sensitivity,specificity,positive and negative predict value,accuracy rate in diagnosing primary hepatic carcinoma by CEUS and contrast-enhanced CT were 97.2%,92.1%,94.5%,96.1%,96.7% and 93.4%,94.1%,95.1%,90.8%,94.1%.CEUS and contrast-enhanced CT were highly consistent with results of pathology（Kappa=0.92,0.86）.Conclusion It is important clinical practical value of CEUS to differential diagnosis of focal liver lesions in fatty liver patients.
