摘要
目的分析我院采取干预措施前后,3种清洁手术围手术期预防用抗菌药物的使用情况,为临床抗菌药物的合理使用提供参考。方法依据《抗菌药物临床应用指导原则》,采用回顾性调查方法,分别对我院3种清洁手术(甲状腺手术,乳腺手术,疝气修补术)干预后107份病例和干预前78份病例进行分析,采用Excel2003进行数据录入和处理,利用SPSS13.0分别对用药持续时间和抗菌药物使用费用作非参数检验。结果抗菌药物平均用药持续时间从7.67天下降为4.94天,经秩和检验有统计学意义;切口感染率差异无显著性;第一代头孢菌素使用比例加大;人均抗菌药物总费用占住院药品总费用的比例由35.37%降至19.21%,抗菌药物使用费用的秩和检验结果有统计学意义。结论我院对3种清洁手术预防用抗菌药物采取干预措施后,抗菌药物的使用现状得到了明显的改善。但抗菌药物使用率高、使用时间长等现象仍然存在。
Objective To analyze the preventive use of antibacterials in three kinds of cleaning operations before and after the hospital interventions,and to provide the reference for clinical rational use of antibacterials. Methods A retrospective investigation method was used to analyze 107 cases after intervention and 78 patients before intervention executing three kinds of clean surgeries （thyroid surgery,breast surgery,hernia repair） in the hospital respectively. The data were put in and managed by Microsoft Excel 2003,the medication persistence time and the cost of antimicrobial drugs were calculated using non-parametric test by SPSS 13.0 software. Results The average medication persistence time was dropped from 7.67 d to 4.94 d,showed statistically significant differences by rank sum test. There was no significant difference in incision infection rates. The usage ratio of the first-generation cephalosporin was increased. The per capita total cost of antibacterials accounted for the total cost of hospital drugs decreased from 35.37% to 19.21%. There was statistical significance of antimicrobial drug usage costs by rank sum test. Conclusion The usage condition of antimicrobial agents has been markedly improved after the hospital taking interventions for preventive use of antimicrobial agents in the three kinds of cleaning operations. But the phenomena such as high antimicrobial drug utilization ratio and the long usage time is still exist.
