摘要
目的探讨白蛋白对伴有低白蛋白血症的老年危重患者的影响。方法将100例血浆白蛋白低于30 g/L的老年危重患者分为2组,在常规治疗及营养支持基础上,治疗组给予20%的白蛋白治疗。观察2组治疗第1天、第7天的APACHEⅡ评分、器官功能评分(Marshall评分)及28 d病死率。结果对照组白蛋白水平不随时间改变,治疗组的白蛋白水平明显升高(P<0.001)。治疗组较对照组的APACHEⅡ评分、Marshall评分有明显改善(P均<0.001),但28 d病死率无明显降低。结论白蛋白治疗可以改善伴有低白蛋白血症老年危重症患者的器官功能。
Objective It is to investigate the effect of albumin on elderly critically ill patients,with hypoproteinemia. Methods 100 elderly critically ill patients with serum albumin less than 30g/L were divided into two groups. On the basis of conventional treatment and nutritional support, the treatment group was given 20% albumin. APACHE Ⅱ scores, organ scores （Marshall score） in the first day and seventh day of treatment and 28 day mortality were observed in both groups. Results The albumin level in the control group did not change with time, and the level rose up significantlily（P 〈 0. 001 ） in the treament group. The improvement of APACHE Ⅱ score and Marshall score in treatment group were better than that in control group （P 〈 0. 001 ） , but there was no significant difference in 28-day mortality between the two groups. Conclusions Treatment with albumin can improve the organ function in critically ill elderly patients complicating with hypoalbuminemia.
