摘要
通过检测果糖胺(fructosamin,FRA)与即刻血糖(bloodglucose,BG)、糖化血红蛋白(glycohemoglobinA1c),HbA1c相关性的研究,确立FRA在胰岛素依赖性糖尿病(IDDM)管理中的重要作用,拟建立和改进糖尿病的监测方法,以“近期、纵向、全程”观察的方法对IDDM进行监护,用以提高糖尿病管理的质量。应用酮胺结构在碱性环境中与硝基四唑氮蓝(NBT)发生还原反应,生成甲月替,并以1-脱氧-1-吗啉果糖(DMF)为标准参照物进行比色测定的方法,对69例血清白蛋白正常的IDDM病人进行为期4个月的监测。每2周测定BG、FRA,每月测定HbA1c,并以30名正常儿童为对照组,求出三者之间的相关关系。FRA与2周前的空腹BG呈良好的正相关(r=1.0395,P<0.05);与HbA1c也有良好的正相关(r=0.9034,P<0.05)。FRA作为短期血糖水平的指标用于糖尿病管理中,可与BG、HbA1c反映不同时段的血糖控制情况,可以把一次检查得到的即时信息,扩展到近一时间段血糖控制的信息,更精确地总结和指导糖尿病的治疗方案。根据研究结果,本文认为建立和改进的监测方法是切实可行的,?
In order to determine the value of FRA in monitoring IDDM,the relationship of FRA with BG and HbA1c was studied in 69 IDDM patients with normal serum protein level and 30 normal children.The duration of monitoring was four months.Every two weeks BG and FRA level were determined and HbA1c level was determined each month.The results showed that FRA had good positive relationship(r=1.0395,P<0.05)with the fast BG level tested 2 weeks ago.It also showed good positive relationship(r=0.9034,P<0.05)with HbA1c.Thus as an index of shortterm blood glucose level,FRA can be used in the monitoring of IDDM.The combination test of FRA,BG and HbA1c can provide useful information for the blood glucose level of a period.
