摘要
目的为探讨Ｍｉｒｉｚｚｉ综合征在临床上的一种新的、合理的和规范的分类方法。方法对２４例此征病人从诊断和治疗的角度加以分析，结合以往对Ｍｉｒｉｚｚｉ综合征的分类方法，重新总结出一种新的分类方法，得出三种不同的类型：Ⅰ型，由于胆囊管或胆囊颈部嵌顿的结石压迫肝总管或者堵塞肝总管而致肝总管狭窄为特点；Ⅱ型，由于胆囊管或胆囊颈部结石压迫肝总管，形成胆囊、肝总管瘘；Ⅲ型，反复的胆囊炎、胆管炎，以及其它良性疾病引起肝管狭窄，上段胆管扩张。结果Ⅰ型１６例，Ⅱ型２例，Ⅲ型６例，三种类型根据临床表现特点和胆管造影可以区别开来。结论对不同类型的Ｍｉｒｉｚｚｉ综合征作不同的处理和治疗。
Objective For discussing a new reasonable and canonical classification of Mirizzi syndrome in surgical clinic.Methods Analysed 24 cases from the view of diagnosis and treatment,and combined the previous classification.A new classification of Mirizzi syndrome was summarized renewedly.There are three types in all.Type Ⅰ is characterized by stenosis of the common hepatic duct due to a stone impacted in the cystic duct or the neck of the gallbladder.Type Ⅱ is characterized by fistula of cystic and the common hepatic duct.Type Ⅲ is characterized by hepatic duct stenosis,the upper bile duct dilatation due to cholecystitis and cholangitis repeatedly and the other benign diseases.Results Of our 24 patients,there were 16 type Ⅰ,2 type Ⅱ and 6 type Ⅲ.These three types were identified by clinical characteristics and cholangiography.Conclusion To treat them differently according to each of these three types.
