摘要
目的探讨血管情况较为特殊的患者在B超引导下经外周中心静脉置管（peripherally inserted central catheter,PICC）的经验。方法 2005年10月-2008年8月对100例经反复置管均未成功的患者,严格无菌条件下,将美国巴德公司（BARD）PICC导管在美国强生公司SONOACE6000C型B超引导下置入。结果 100例B超引导下穿刺全部成功,穿刺成功率为100%。98例置管成功,成功率98%。1例因腋静脉畸形置管20cm后无法送管到预定长度,1例来我院时已分别在外院经外周静脉化疗4-6个周期,血管内膜损伤严重,穿刺成功后无法送管。结论 B超引导下行PICC,为PICC置入困难的患者提供了可行手段。
Objective To summarize our experience in the PICC for patients whose special blood vessels under the guidance of B-ultrasonography.Methods From October 2005 to August 2008,totally 100 patients who failed in PICC were enrolled into this study.Under strict aseptic condition,PICC（BARD Co.,USA）were inserted under the guidance of B-ultrasonography（SONOACE6000C,JNJ Co.,USA）.Results In our patients,the success rate of puncture was 100%,and that of PICC insertion was 98%.In the two patients who failed,malformation of the axillary artery was detected when 20 cm catheter was placed,and the other had severe injury to the vascular endagium after 4 to 6 cycles of chemotherapy via peripheral veins.Conclusions For difficult cases for PICC insertion,B-ultrasonography can be used for guidance.
