摘要
目的介绍上海市降低宫颈癌发病率及提高诊治质量的经验。方法通过子宫卵巢癌专题协作组，定期开展宫颈癌防治工作：①调查宫颈癌的高危因素；②探索普查优选方案；③分析宫颈癌误诊原因；④每年统计宫颈癌的标化率；⑤比较１９９５年妇科三癌（宫颈、子宫、卵巢癌）的标化率。结果①宫颈癌的发病年龄５０岁以上者占９５．８％。②提高了读片质量。③１９９８年共误诊５例，延误就诊了３例，漏诊２例。④宫颈癌标化率从１９７８～１９８２年的１１．８／１０万，下降到１９９５年的２．１／１０万。⑤１９９５年妇科三癌的标化率分别为：宫颈癌２．１／１０万，子宫内膜癌４．４／１０万，卵巢癌６．４／１０万。结论改进普查方法，提高了普查质量，达到了早防、早诊及早治。
Objective Present the experience of bringing down the incidence of cervix uteri cancer and improving the quality of diagnosis and therapy. Methods Establish a cooperative research group of uterine and ovarian cancers and carry out prophylaxis and treatment periodically. (1) Study the high risk factors of cervix uterine cancer. (2) Optimize the plan of general survey. (3) Find out the causes of miss diagnosed cases. (4) Survey the standardized incidence of cervix uterine cancer in the residents of Shanghai from 1978 1995. (5) Compare the standardized incidence of cervix uterine cancer, endometrial cancer and ovarian cancer in 1995.Results (1) According to the age incidence survey, most patients (95.8%) are over 50. (2) Promote the technique of examination of Rap smears. (3) there were 5 cases misdiagnosed in 1988, 3 cases were delayed by patients themselves and 2 cases were missed by doctors. (4) The standardized incidence of cervix uteri cancer from 1978 1982 was 11.8/100,000 and dropped to 2.1/100,000 in 1995. (5) The standardized incidence of cervix uterine cancer, endometrial cancer and ovarian cancer in 1995 were 3.5/100,000, 4.8/100000 and 8.7/100000 respectively. Conclusion Improvement of methodology and quality of general survey will promote the early prevention, early diagnosis, early treatment and ultimately lowered the incidence of cervix uterine cancer in Shanghai.
