摘要
目的:研究一支冠脉多次短暂缺血发作能否使另一支冠脉灌注区心肌在随后的持续缺血中得到保护。方法:13只家兔随机分为缺血预处理组和对照组。缺血预处理组(7只):前降支阻闭/再灌注各5min,共4次,随后阻闭左室支45min、再灌注2h;对照组(6只):旷置40min后,阻闭左室支45min、再灌注2h。结果:缺血预处理组心梗面积明显小于对照组(892%±17%vs14.09%±193%,P<001),两组间心率、左室收缩压及dp/dt、动脉收缩压和舒张压均无显著差异(P>005)。结论:局部缺血“预处理”能保护兔相邻部位持续缺血心肌,减少心肌梗塞面积;缺血预处理不能直接改善兔心持续缺血后心脏功能。
AIM: To access whether brief occlusions of one coronary artery also protect near-by, virgin myocardium from subsequent sustained coronary artery occlusion in rabbit model. METHODS: 13 rabbits were randomized to undergo either four episodes of 5-min left anterior descending coronary (LAD) occlusion, 5-min reperfusion (preconditioned group, n =7) or no intervention (control group, n =6) before 45-min occlusion of the large marginal branch of the circumflex artery.RESULTS: The infarct size in preconditioned group was significantly less than that in the control (8.92%±1.7% vs 14.09%±1.93%, P <0.01). But the heart rates, left ventricular systolic pressures (LVSP) and d p /d t , and blood pressures were similar ( P >0.05) in both groups. CONCLUSION: Brief episodes of ischemia in one vascular bed may limit infarct size in another near-by, virgin myocardium from subsequent sustained coronary artery occlusion in rabbit model; regional ischemic preconditioning does not, however, show an independent beneficial effect on ventricular contractile function.
