端粒酶hTR基因在石蜡包埋肝脏活检组织中的原位杂交检测

DETECTION OF TELOMERASE hTR IN FORMALIN FIXED PARAFFIN EMBEDDED LIVER TISSUE BY IN SITU HYBRIDIZATION

摘要 目的:探讨hTR与肝细胞癌发生的关系,及与肝炎病毒感染的关系,扩大小样本检查在临床的应用,探讨端粒酶hTR在肝细胞癌活检组织中的活性,并与手术切除的肝癌组织进行了比较。方法:62例肝组织标本,其中42例肝脏活检穿刺标本(16例肝细胞癌,14例肝硬化,12例慢性肝炎),18例手术切除的肝癌标本,2例尸解正常肝脏组织,均做了hTR的活性检测。结果:外科手术切除的癌组织中,hTR活性表达72.2%(13/18),在针吸活检肝组织中,hTR活性表达81.3%(13/16),在高分化癌中77.8%(7/9),中分化癌中91.67%(11/12),低分化癌中62%(8/13),在慢性肝炎、肝硬化中53.8%(14/26)。60例标本中有26例病毒阳性标记,其中在病毒阳性及阴性标记的肝癌组织中hTR阳性表达均为83.3%(10/12),在病毒标记的慢性肝炎、肝硬化中hTR活性表达50%(7/14),在肝癌旁组织中hTR阳性表达8.3%(2/24)。结论:端粒酶hTR阳性表达与肝细胞癌的发生有关,与肝炎病毒标记状态无关,针吸活检标本在临床慢性肝脏疾病中,在小的不同类型结节性损伤的诊断中,端粒酶的分析是可行的。 Objective:To study relation between the expression of hTR gene and hepatitis viral infection in hepatocellular carcinoma (HCC) and chronic hepatic lesions,and to extend the small sample test to clinical use.we detected telomerase activity in human liver biopsied samples,and compared them with HCC tissues surgically resected.Methods:Forty two liver samples (from 16 HCC,14 liver cirrhosis,12 chronic hepatitis) that were biopsied,18 HCC samples that were surgically resected,and 2 normal liver tissue samples from autopsy were analyzed for telomerase hTR.Results:hTR activity expression occurred 72.2% (13/18) in HCC (surgically resected),and 81.3% (13/16) in biopsied samples.The hTR expressions were found 77.8% (7/9) in 91.67% (11/12) in moderately differentiated HCC,62% (8/13) in poorly differentiated HCC,and 53.8% (14/26) in chronic cirrhosis and hepatitis.The same 83.3% (10/12) expression was found in the HCC samples with hepatitis virus positive or negative.In the virus marked chronic hepatitis and cirrhosis,hTR activity expression was up to 50% (7/14);and positive hTR expression in pericarcinomatous tissue was 8.3% (2/24).Conclusion:Telomerase hTR positive expression relates to HCC genesis,but does not relate to viral marker status.It is feasible to analyse telomerase hTR in cases of chronic hepatopathy or during diagnosing some small nodular lesions.
