摘要
目的:检测儿童急性白血病血脂水平,探讨其与病情严重程度、疗效和预后的关系。方法:用酶学法测定空腹晨血血清总胆固醇(T-Ch)、游离胆固醇(F-Ch)、胆固醇脂(Che)、甘油三脂(TG)、磷脂(PL)、载脂蛋白A1(apoA1)和载脂蛋白B(apoB)。结果:化疗前组T-Ch、Che、PL、apoA1比对照组和完全缓解组明显降低,而F-Ch、TG、apoB变化无意义。完全缓解组七项指标与对照组相比无差异。急性淋巴细胞白血病与急性非淋巴细胞白血病血脂水平无差别。外周血白细胞≥20×109/L组,PL较<20×109/L组低。疗效差的病人T-Ch、PL虽有恢复但apoA1持续降低。结论:儿童急性白血病存在低脂血症,动态观察血脂水平特别是PL、apoA1对了解白血病细胞负荷、估计疗效有帮助。
Objective:To investigate the relationship of the serum lipids with disease activity and prognostic relevance through the determination of the serum lipids levels in children with acute leukemia Method:Serum total cholesterol (T Ch),free cholesterol (F Ch),cholesterolester (Che),triglyceride (TG),phospholipid (PL),apolipoprotein A 1 (apo A 1) and apolipoprotein B (apo B) were determined by using enzymatic methods Results:The levels of T Ch,Che,PL,apo A 1 in children with acute leukemia before treatment were significantly lower than those in children of complete remission (CR) after chemotherapy and those in the normal controls But the levels of F Ch,TG,apo B among the three groups had not marked differences No significantly variation of the levels of the seven lipid parameters was found between the children of CR and the normal controls The levels of lipids of children with acute lymphocytic leukemia and those with acute nonlymphocytic leukemia had no differences The level of PL in group of blood WBC ≥20×10 9/L was lower than that in group of WBC <20×10 9/L A further decrease of apo A 1 was found in three patients who had bad prognosis Conclusion:Hypocholesterolemia is common finding in children with acute leukemia Serum lipids,especially PL and apo A 1 may be of value in assessing tumor cell burder,disease activity and prognosis
