摘要
目的:研究羊水过少窒息新生儿脐动脉血乳酸水平的变化及与血气分析新生儿结局的关系。方法:回顾性分析足月单胎头位羊水过少产妇Apgar评分≤7分新生儿57例(羊水过少组)临床资料,并随机抽取同期分娩羊水正常产妇Apgar评分≤7分新生儿57例作对照(羊水正常组),探讨脐动脉血乳酸、血气分析及其新生儿行为神经测定(Neonatal behavioral neudogio-alassessment,NBNA)间关系,所有病例均无其他病理因素。结果羊水过少组与羊水正常组脐动脉血乳酸水平、pH值及碱剩余(BE)比较差异有统计学意义(P<0.05)。脐动脉血乳酸与pH值、BE呈明显负相关关系(r=-0.64和-0.63,P<0.05)。新生儿NBNA评分轻度窒息两组间相比无统计学意义(P>0.05),重度窒息有统计学意义(P<0.05)。结论:脐动脉血乳酸水平与pH值、BE有较好的相关性,羊水过少与提示急性窒息的指标脐动脉血乳酸有明显关系,窒息程度与新生儿NBNA评分关系密切。
Objective T0 determine whether umbilical artery lactate level were significantly different in infant with or without intraparturn asphyxia and hypanmion. Method For 114 term parturients with cephalia precentation, singleton fetus, without obstetric and medical complications, newborn infant Apgar score ≤7 were retrospectively analyzed,in which there were 57 cases in hypamnion group and 57 cases in normal amniotic fluid group. The lactate level and acid - base balance were measured at each delivery. At 4 - 6th and 26 -28th days after birth,neonatal behavioral neudogioal assessment (NBNA) was taken for each newborn. Results There were significant differences in mean lactate level, pH value and BE in newborns with hypamnion compared with normal amniotic fluid group( P 〈 0.05 ). The lactate levels in hypamnion group was negatively correlated with ti,e pH value and BE (r = -0.64 and -0. 63 ,respectively,P 〈0. 05). There was no difference of NBNA score at the 4 - 6th and 26 -28th days after birth with light - degree asphyxia between hypamnion group and normal amniotie fluid group ( P 〉 0. 05 ). But there were significant differences in serious asphyxia between the two groups ( P 〈 0. 05 ). Conclusion This study demonstrates that there are good correlation in umbilical artery lactate levels, pH value and BE in newborns,and there is obvious relation between hypamnion and lactic acid, which is the index of acute asphyxia. There is closely relation between NBNA score and asphyxia degree.
