摘要
目的探讨腹内疝的术前CT诊断标准,提高对本病影像学诊断的认识。方法回顾性分析15例腹内疝患者的临床及CT资料。结果 15例术前CT检查均表现为小肠梗阻,6例发生肠管绞窄。10例考虑腹内疝,其中网膜孔疝1例,十二指肠旁疝2例,大网膜裂孔疝1例,盲肠周围疝1例,肠黏连束带疝5例。术后证实均有肠梗阻,其中,十二指肠旁疝2例,网膜孔疝1例,大网膜裂孔疝2例,盲肠周围疝1例,胃切除术吻合口后疝1例,肠黏连束带内疝8例。各型腹内疝CT的共有征象和一系列个体征象与手术病理结果存在相关性。结论多层螺旋CT能够较好地诊断腹内疝,有效指导手术方案的选择。
Objective To discuss the CT diagnosis criteria of internal abdominal hernia,and improve understanding of this disease. Methods The clinical and CT data of 15 patients were analyzed retrospectively. Results In all of 15 cases,who were diagnosed as intestinal obstruction,CT showed strangulation of intestinal in 6 cases. 10 cases were diagnosed as internal abdominal hernia,including 1 case of foramen of Winslow hernia,2 cases of paraduodenal hernia,1 case of pericecal hernia,1 case of greater omentum hiatus hernia,and 5 cases of internal abdominal hernia induced by accretio after operation. There were 8 cases of strangulation of intestine in all of 15 intestinal obstruction cases which were confirmed after operation. 1 case of foramen of Winslow hernia,2 cases of paraduodenal hernia,1 case of pericecal hernia,2 cases of greater omentum hiatus hernia,and 9 cases of internal abdominal hernia induced by accretio after operation or postgastrectomy. Common and individual CT signs of all kinds of internal abdominal hernia correlated with results of operation and pathology.Conclusions CTis valuable in diagnosis of internal abdominal hernia and guiding thesurgical programs.
