摘要
目的比较改良塞丁格技术与传统PICC置管在化疗患者中的应用,并分析其利弊。方法 56例经改良塞丁格技术穿刺的经外周静脉置入中心静脉导管(peripherally inserted central catheter,PICC)患者为观察组,73例以传统PICC置管的患者为对照组,记录置管适用率、置管成功率及相关并发症。结果观察组肘上、肘下置管适用率分别为96%、100%,对照组分别为11%、66%(P<0.01);观察组一次置管成功率为93%,对照组为52%(P<0.01);观察组相关并发症发生率4%,对照组为11%(P<0.01);采用塞丁格技术置管的费用平均每次要高于传统PICC 500元。结论采用改良塞丁格技术置入PICC提高了置管适用率、置管成功率,降低了PICC并发症发生率,患者痛苦小,值得临床推广应用。但传统PICC置管因价格优势,对血管条件较好的患者仍不失为一种可选择的化疗置管方法。
Objective To explore the applications of modified seldinger and traditional PICC catheterization in patients undergoing chemotherapy and analyze their advantages and disadvantages.Methods There were 56 patients applied modified seldinger in experimental group,and 73 cases with traditional PICC catheterization in control group.The catheter indications,success rate of catheterization and main complications were recorded in two groups.Results The catheter indications and the success rate of catheterization in experimental group were 96%-100% and 93% respectively,higher than those in control group(P〈0.01);while in experimental group,complication rate accounted for 4%,lower than that in control group(P〈0.01);however,modified seldinger technology cost 500 yuan more than traditional PICC.Conclusion With the application of modified seldinger technology,catheter indications and the success rate of catheterization have been improved and the complication rate has been reduced.However,patients with better vascular conditions can choose the traditional PICC catheterization for its advantage over price.
