低温等离子射频消融和微波治疗慢性肥厚性鼻炎疗效分析 被引量:22

Comparison of Curative Effect between Radiofrequency Ablation and Microwave for the Treatment of Chronic Hypertrophic Rhinitis

摘要 目的探讨低温等离子射频消融和微波治疗慢性肥厚性鼻炎的临床疗效。方法我院近3年诊治慢性肥厚性鼻炎患者168例,分为低温等离子组和微波治疗组。观察患者术后3个月及6个月时手术疗效,疼痛视觉模拟(VAS)评分法评价术前及术后3、6个月的鼻通畅情况,观察术后水肿、脱痂时间,有无瘢痕化的黏膜形成及鼻干、鼻出血、涕倒流等并发症。结果低温等离子组术后3个月时总有效率为96.2%;6个月时总有效率为93.6%。微波治疗组术后3个月时总有效率为95.5%;6个月时总有效率为92.1%。两组总有效率比较差异均无统计学意义(P>0.05)。两组术后VAS评分比较差异无统计学意义(P>0.05)。两组术后水肿时间及脱痂时间比较差异均无统计学意义(P>0.05)。低温等离子组术后鼻腔黏膜瘢痕形成、鼻干、涕倒流发生率较微波治疗组明显降低,差异有统计学意义(P<0.05)。结论低温等离子射频消融和微波皆可有效治疗慢性肥厚性鼻炎,低温等离子消融并发症少,可优先采用。 Objective To investigate the clinical effect for treating chronic hypertrophic rhinitis by using radiofrequency ablation technology.Methods Totally 168 patients with hypertrophic rhinitis(HR)been treated in recent 3 years in our hospital were divided into two groups.The patients in one group(n=79) were treated by radiofrequency ablation,and those in the other group(n=89) were treated by microwave.The efficacies in the two groups were observed 3 months and 6 months after the operation.The visual analogue pain scale(VAS) was used too assess the condition of the nasal smoothness.Results The total effect rate in the radiofrequency ablation group was 96.2% 3 months after the operation and 6 93.6% 6 months after the operation,as compared with 95.5% and 6 92.1% in the microwave(P0.05).In VAS score was no significant difference between the two groups(P0.05).There was no significant difference between the two groups in postoperative decrustation time and edema time(P0.05).But the incidences of nasal mucous cicatrization,nasal dryness and postnasal drip were higher in the radiofrequency ablation group(P0.05).Conclusion Both radiofrequency ablation and microwave are effective in the treatment of chronic hypertrophic rhinitis.And the radiofrequency ablation technique should be first adopted with its less complication.
