儿童过敏性紫癜血浆D-二聚体和纤维蛋白原降解物检测的临床意义及应用 被引量:13

摘要 目的研究过敏性紫癜(HSP)患儿急性期D-二聚体、纤维蛋白原降解物(FDP)、血浆凝血酶原时间(PT)、部分凝血活酶时间(KPTT)的变化,了解其血液的高凝和纤溶亢进的变化。方法对35例急性期HSP患儿及16例健康儿童采血,采用胶乳凝集试验检测D-二聚体和FDP,并测定PT、KPTT。结果 HSP患儿35例,D-二聚体检测阳性13例,FDP检测阳性12例,阳性率分别为37.1%和34.3%;对照组16例中,D-二聚体和FDP检测均为阴性。两组D-二聚体、FDP阳性率比较差异均有统计学意义(χ2=7.98和7.17,P<0.01);PT和KPTT水平比较差异无统计学意义(t=0.422和0.478,P>0.05)。结论 D-二聚体和FDP的升高是HSP高凝和纤溶活性增强较灵敏的标志,其检测对合理应用抗凝治疗及改善预后等有重要意义。 Objective To explore the changes of acute-phase D-dimer,fibrinogen degradation products(FDP),plasma prothrombin time(PT),partial thromboplastin time(KPTT) to contribute the knowledge of blood hypercoagulation and hyperfibrinolysis in children with Henoch-schonlein purpura(HSP).Methods Blood samples were collected from 35 acute HSP children and 16 healthy.D-dimer,FDP were determined by Latex Agglutination Tests,and PT KPTT detected.Results Of 35 HSP children,13 had positive D-dimer,12 had positive FDP,the positive rates were 37.1%,34.3%,respectively;in control group,D-dimer and FDP were negative.there was significant difference in D-dimer and FDP between 2 groups(χ2=7.98,7.17,P0.01),but there was not in PT and KPTT(t=0.422,0.478,P0.05).Conclusion Elevated D-dimer and FDP,sensitive markers for HSP hypercoagulation and enhanced fibrinolysis activity,are of significance in rational application of anticoagulant therapy and improvement of prognosis.
