盐酸氨溴索预防体外循环术后急性肺损伤的作用与机制 被引量:5

摘要 目的研究体外循环手术前使用盐酸氨溴索对体外循环手术后急性肺损伤的保护机制。方法将80例体外循环手术患者随机分为4组(空白对照组以及5、10、20 mg/kg盐酸氨溴索组),分别给予生理盐水及相应分组剂量盐酸氨溴索,测定术后气道吸取物中磷脂含量及血中SOD、MDA含量。结果气道吸取物中磷脂含量及血中SOD、MDA含量在实验组与对照组间差异有统计学意义(P<0.05),大剂量盐酸氨溴索组(20 mg/kg)与中剂量组(10 mg/kg)差异无统计学意义(P>0.05),但与空白对照组、小剂量组(5 mg/kg)间差异有统计学意义(P<0.05)。结论体外循环手术前使用盐酸氨溴索可以促进肺泡表面活性物质的合成,减轻体外循环手术后急性肺损伤,这种保护作用在一定剂量范围内存在剂量依赖性。 Objective To study the mechanism of pretreatment with ambroxol against acute lung injury after cardiopulmonary bypass surgery.Methods Eighty patients with cardiopulmonary bypass were randomly divided into 4 groups(control group,as well as 5,10,20 mg/kg ambroxol groups) and were given saline and the corresponding sub-dose ambroxol.Phospholipid content in tracheal aspirate and serum SOD and MDA were determined postoperative.Results Significant differences in phospholipid in tracheal aspirate and serum SOD and MDA in were observed between ambroxol groups and control group(P〈0.05).Though there was no significant difference in these parameters observed between high-dose amborxol group(20 mg/kg) and medium-dose amborxol group(10 mg/kg)(P〉0.05),there were significant differences between high-dose ambroxol and control and low-dose ambroxol groups(5 mg/kg)(P〈0.05).Conclusion Pretreatment of ambroxol in cardiopulmonary bypass surgery can promote surfactant synthesis and reduce postoperative acute lung injury in a dose-dependent manner within a certain range.
