椎旁注射阿霉素对大鼠肌力和坐骨神经的影响

摘要 目的研究大鼠腰椎旁注射不同浓度阿霉素(ADM)后,大鼠后肢肌力和运动状态的改变及其对坐骨神经的毒性作用。方法 80只大鼠随机均分为四组:0.33%ADM组(Ⅰ组),0.5%ADM组(Ⅱ组),1%ADM组(Ⅲ组),生理盐水对照组(Ⅳ组),分别向右侧腰椎间孔(L4、L5)注射0.33%、0.5%、1%ADM、生理盐水10μl。术后1、2、4、8周对每组大鼠的右下肢肌力及运动状态进行评价,并于术后2、4周每组随机取2只大鼠进行坐骨神经的组织学观察。结果 III组术后1~8周大鼠右下肢出现不同程度的运动障碍,肌力评分明显低于其它三组(P<0.05)。相应的坐骨神经纤维细胞中可见大量坏死改变。Ⅱ组出现重度运动障碍,坐骨神经纤维细胞有明显的坏死性改变。Ⅰ组坐骨神经未见明显病理改变。Ⅳ组有数只大鼠在初期出现了轻微的运动障碍,但后期恢复正常。其坐骨神经未见病理改变。结论 0.33%ADM属于临床上应用的安全浓度。 Objective To study the effect of adriamycin paravertebral injection on muscle power and sciatic nerve in rats. Methods Eighty rats were randomly divided into four groups and injected 10μμ of 0. 33%(group A), 0. 5%(group B), 1 %(group C) of adriamycin or normal saline(group D) in the right side of L4, LS. The right lower extremity muscle force and motion state were evaluated on the 1s5, 2nd, 4th and 8th week after injection, respectively. The morphologic changes of the sciatic nerve were observed on the 2nd or 4th week after operation. Results The ipsolateral posterior limbs of rats displayed motor disturbance to different degrees in group C from 1 to 8 weeks and the scores of muscle force were obviously lower than those in group D(P〈0. 05). A large number of necrotic cells was found in the sciatic nerve in grorup C. Dyskinesia of posterior limbs was found in several rats in group B. But obvious pathological changes in their sciatic nerve cells were found in group B, which was not seen in group A. In group D, a few rats showed a slight motor obstacle in the initial stage, but returned to normal later, and no pathological changes were seen in the sciatic nerves. Conclusion Adriamycin 0. 33% is a safe concentration for clinical application.
