C反应蛋白与脑梗死患者颈动脉粥样硬化病变的关系 被引量:10

The Relationship Between C-Reactive Protein and Carotid Atherosclerosis in Cerebral Infarction

摘要 目的探讨C反应蛋白与脑梗死患者颈动脉粥样硬化和预后的关系。方法筛选94例首次发病的急性脑梗死患者,应用免疫比浊法测定血清C反应蛋白水平,应用颈动脉超声评价颈动脉粥样硬化的特征,并对患者神经功能缺损评分、残疾率和死亡率进行评价。结果与低C反应蛋白组脑梗死患者相比,高C反应蛋白组脑梗死患者颈动脉内膜中膜厚度和不稳定性斑块的发生率显著增高(P<0.05);高C反应蛋白组脑梗死死亡率、残疾率和神经功能缺失评分均显著高于低C反应蛋白组(P<0.01)。结论 C反应蛋白可以反映颈动脉粥样硬化的特征和斑块的稳定性,是了解脑梗死预后的重要指标。 Aim To investigate the relationships between C-reactive protein and carotid atherosclerosis and prognosis in patients with cerebral infarction. Methods 94 patients with first cerebral infarction were selected.The serum level of C-reactive protein was determined by immunonephelometric assay.The features of carotid atherosclerosis was assessed by carotid ultrasonography.The neurological deficit scores,mortality and disability rate were evaluated in patients with cerebral infarction. Results The carotid intima-medial thickness and the incidence of carotid unstable plaque in high C-reactive protein group were significantly higher than those of low C-reactive protein group in patients with cerebral infarction(P0.05);The neurological deficit scores,mortality and disability rate in high C-reactive protein group were significantly higher than those of low C-reactive protein group in patients with cerebral infarction(P0.01). Conclusion The serum level of C-reactive protein may reflect the character and plaque stability of carotid atherosclerosis in patients with cerebral infarction,which may bea prognostic marker in patients with cerebral infarction.
