摘要
目的探讨神经内镜下经鼻蝶窦入路在垂体腺瘤切除术中的应用。方法应用神经内镜及其辅助的工作系统进行手术切除垂体腺瘤19例。其中泌乳素腺瘤7例,生长激素腺瘤6例,无功能腺瘤6例。结果 19例患者中,全切除肿瘤17例,近全切除肿瘤2例。术后症状均有不同程度的改善。无鼻中隔穿孔、萎缩性鼻炎、脑脊液漏及颅内出血等并发症发生。19例患者随访6～18个月,肿瘤无复发。结论神经内镜下经鼻蝶窦入路具有创伤小、术中暴露充分、全切率高及术后并发症少等优点,是垂体腺瘤切除的理想术式。
Objective To explore the article of clinic application of the single-nostril transsphenoidal endoscopic approach in the treatment of pituitary adenomas.Methods Nineteen patients with pituitary adenomas were treated through single-nostril transsphenoidal endoscopic approach.7 cases with prolactinomas,6 cases with growth-secreting adenomas,and 6 cases had nonfunctional adenomas.Results Among 19 patients,total removal was achieved in 17 cases,major removal in 2 cases.Postoperative symptoms were improved.The nasal septum perforation,atrophic rhinitis,cerebrospinal fluid leak and intracranial bleeding.no severe complications was found.19 patients were followed up for 6 to 18 months,no tumor recurrence.Conclusion The single-nostril transsphenoidal endoscopicprovides an less invasive therapy for pituitary adenomas,and can expose fully in operation.This approach is safe and eficient to remove adenomas completely without severe complications.It is an ideal method in the treatment of pituitary adenoma.
