摘要
目的:探讨应用超声心动图诊断胎儿心律失常的价值。方法:对白云区妇幼保健院2007年6月-2009年3月1 390例门诊孕妇中,通过超声检查、胎儿心电图或胎心监护疑有胎儿心律失常的158例患者进行胎儿超声心动图检查,对胎儿心律失常者进行干预治疗。结果:通过心脏彩超共诊断出68例胎儿心律失常,其中房性早搏16例,室性早搏8例,心动过速37例,心动过缓7例。其中2例患有心动过速的胎儿合并有心脏畸形,均为室间隔缺损;1例心动过缓胎死宫内,其余均不伴有先天性心血管畸形或其他畸形,出生后听诊均未闻及明显心脏杂音或心律不齐。本组患者均行鼻导管低流量给氧,同时经静脉给药,予能量合剂。结论:胎儿心律失常为胎儿期最常见的非结构性异常,多不伴有明显心血管畸形或其他胎儿畸形,绝大多数预后良好。超声心动图可以快速有效的检测出胎儿心律失常,而且可以较为准确的对胎儿心律失常进行分类,是胎儿心律失常的首选诊断措施,行吸氧加能量合剂宫内治疗效果好且无致畸作用。
Objective:To explore the value of the application of fetal echocardiography in the diagnosis arrhythmia.Methods:From June 2007 to March 2009 in our hospital,158 cases among 1390 cases of out-patient clinics for pregnant women were detected by fetal echocardiography,who worried about fetal arrhythmia by ultrasound examination,fetal heart rate monitoring and fetal ECG.Meanwhile,the fetal cardiac arrhythmia received intervention therapy.Results:A total of 68 cases of diagnosed fetal arrhythmia were found by Color Doppler,of which 16 cases of atrial premature beats,8 cases of premature ventricular contractions,37 cases of tachycardia,7 cases of bradycardia.2 cases of fetal tachycardia with heart malformations were associated with both ventricular septal defect;1 cases of intrauterine fetal died because of bradycardia,and the rest cases were not associated with congenital cardiovascular malformations or other deformities,auscultation after birth were not obviously heard on the heart murmur or arrhythmia.Patients in this group were treated by low-flow nasal oxygen catheter,intravenous administration at the same time associated with the mixture of energy.Conclusion:The fetus of fetal arrhythmia show non-structural abnormalities,most of whom do not show significant cardiovascular malformations or other birth defects and have good prognosis.The echocardiography can detect fetal arrhythmia quickly and effectively and can classified the fetal arrhythmia accurately,so it is the first choice for diagnosis of fetal arrhythmia measures.The intrauterine treatment of oxygen catheter associated with the mixture of energy has good treated effect with no deformities.
