摘要
目的:探讨舒芬太尼延迟性预处理联合早期预处理对大鼠心肌缺血再灌注损伤的影响及可能的机制。方法:健康成年雄性Wistar大鼠50只,随机分为5组(n分别=10):假手术组(S组)、心肌缺血再灌注组(I/R组),舒芬太尼延迟性预处理组(SF1组)、舒芬太尼预处理组(SF2组)、舒芬太尼延迟—预处理组(SF1+SF2组)。缺血再灌注24h前,S组、I/R组和SF2组静脉输注生理盐水20ml,SF1组和SF1+SF2组静脉输注含5μg/kg舒芬太尼生理盐水20ml。采用夹闭左冠状动脉前降支40min后再灌注的方法制备心肌缺血再灌注模型。SF2组和SF1+SF2组在缺血再灌注前15min予舒芬太尼5μg/kg。监测缺血前即刻值和再灌注后30min,60min,120min时心率、平均动脉压,并计算平均动脉压和心率的乘积及检测心肌肌钙蛋白I(cTnI)。测定缺血再灌注120min时半胱氨酸蛋白酶3(Caspase-3)含量、心肌细胞凋亡指数和心肌梗死面积。结果:与缺血前即刻值比较,除S组,各组再灌注各时相心率、平均动脉压以及平均动脉压和心率的乘积均降低(P<0.05),但组间比较差异无统计学意义。与I/R组比较,SF1组、SF2组及SF1+SF2组cTnI、心肌梗死面积、Caspase-3含量和心肌细胞凋亡指数均降低(P<0.05),SF1+SF2组低于SF1组、SF2组(P<0.05),差异均有统计学意义。结论:舒芬太尼延迟性预处理联合早期预处理可减轻缺血再灌注损伤,较单独应用时效果好。
Objective:To investigate the effect of early pre-conditioning and delayed pre-conditioning of sufentanil on myocardial ischemia reperfusion injury in rats. Methods:Fifty Wistar male rats were randomly divided into 5 groups, n = 10 in each group. Sham operation group (S group), Ischemidreperfusion group(l/R group), Sufentainil delayed pre-conditioning group (SF1 group), Sufentainil early pre-condi- tioning group (SF2 group), Sufentainil delayed pre-conditioning with early pre-conditioning group( SF1 + SF2 group). 24 hours prior ischemia reperfusion, the animals received 20 ml of normal saline in S group, L/R group, and SF2 group; the animals re- ceived Sufentainil 5 ~g/kg with 20 ml of normal saline in SF1 group and SF1 + SF2 group intravenously. 24 hours later, I/R was induced by clamping the left anterior descending artery for 40 minutes in I/R and SF groups. 15 minutes prior I/R, SF2 group and SF1 + SF2 group received Sufentainil 5 ~Lg/kg. The heart rate (HR) , mean arterial pressure (MAP) and the product of HR and MAP were monitored at the baseline and post reperfusion 30 rain, 60 min and 120 min respectively; the value of cardiac tro- ponin I (cTnI)was measured as well. Myocardial infarction (MI)size, expression of caspase-3 and apoptotic index (AI)of myo- cardium were measured at the end of 120 min of reperfusion in each group. Resuhs:Compared with the baseline, HR, MAP and the product of HR and MAP were decreased in all SF groups (P 〈0.05 ) during the reperfusion, but no significant difference among different SF groups. Compared with I,/R group, cTnI, MI size, caspase-3 levels and AI were decreased in all SF groups ( P 〈 0.05 ) , while the above values in SF1 + SF2 group was lower than that in SF1 group and that in SF2 group respectively ( P 〈 0. 05 ). Conclusion:Sufentanil delayed pre-conditioning with early pre-eonditioning reduced I,/R injury, the protective effect was better than either Sufentanil early pre-conditioning or Sufentanil delayed pre-conditioning of I/R injury in rats.
