摘要 单肺通气作为麻醉领域内一个比较特殊的通气方式,在为胸外科手术提供便利条件的同时也不可避免地会产生一定程度的肺损伤。为减轻或避免围术期肺损伤,通常采用下列措施,如选择合适麻醉方式、麻醉药物、呼吸参数、通气模式及非通气侧肺持续低压给氧、应用药物改善氧合、通气侧肺的保护、浅低温以及其他相关肺保护措施。本文就近年来单肺通气期间肺损伤的机制及保护策略的研究进展作一综述。 One -lung ventilation is a quite special ventilation way in the anesthesia domain, offering the convenience for the smooth development of thoracic surgery, meanwhile it also inevitably causes lung injury. In order to reduce or avoid lung injury in the operating period, we usually use the following measures: choosing a appropriate anesthesia way and the anesthesia drugs, the suitable breathing pattern and the breathing parameter, ventilation mode, taking the low pressure oxygen into the non - ventilation lung, the applications of drugs to improving the oxygenation, protection of the ventilative lung, mild hypothermia and the other relevant measures to protect the lungs. Now, we make a summary for present application progress of the lung injury mechanism and protection strategy during one - lung ventilation in recent years.
